Brief Answer:
no permanent solution. weight loss helps

Detailed Answer:
Dear XXXXXXX
Thanks for writing to healthcare magic.
Polycystic ovarian syndrome is a multihormone problem.there are problems at multiple levels in this disease.
Some have irregular cycles and mild acne.
Some patients have severe hormone dysfunction and weight gain.
So there is no single solution that suits all.
The treatment depends on the patients needs.
As the patient is 39 and has hopefully completed family the main problem will be to get regular cycles I suppose.
So I recommend that along with weight loss and metformin, tab like ginette 35 helps.
If tr patient is trying to conceive then ovarian drilling helps a lot but I don't recommend surgery just to get regular cycles.
Hope this was helpful.
Thanks
