Suggest Treatment For MOTT Infection Along Chills, Fever And Vomiting

Question: Doctor my wife underwent laproscopy for endometriosis and culture report revealed MOTT infection. And she is taking AKT 4 since 20 days as prescribed by the doctor.
Now she is feeling chill, fever and vomiting. She is also feeling weakness in the body and heavy and drowsy head. She has pain on the right side of the neck with little bit swelling. She is finding everything tasteless and bitter. She is also feeling sleepy. Are these symptoms normal while taking AKT4? Does she need to take any other medication with AKT4? Can she take paracetamol or painkiller during AKT4 treatment? Please suggest us what to do as we reside in Dubai, UAE.

Dear Friend.
Welcome to Healthcaremagic.
Hi, I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.
These symptoms following starting of Anti Tubercular Treatment (ATT) can be due to following causes:
1. Acute hepatitis- Jaundice secondary to ATT intake.
2. Acute Gastritis.
I would advise you to get following tests on urgent basis:
1. Liver Function Test
2. PT/INR
3. Serum Creatinine.
If jaundice is evident on reports, the ATT will be modified.
Also, i would advise to take a combination of Pantoprazole and Domperidone for relief from Gastritis causes by ATT.

Hi.
PT/INR test is done to rule out coagulopathy that can result from deranged liver functions.
If it is raised there are chances of bleeding.
You can first get the LFT done. If it is deranged then you would require PT/INR. I mentioned it and get it done in my patients along with LFT to avoid resampling.

Hi.
Good news is that she doesn't have ATT induced hepatitis.
I would advise following in such scenario:

1. To take a combination of Pantoprazole and Domperidone.
2. To take soft, bland diet.
3. To take anti flatulent and anti acidity syrup like Digene

Thank you doctor. As mentioned in my earlier msg, my wife has swelling of nodes on the right side of neck which has pain. Can she take any pain killers. Awhat is the dosage of medicine given above be taken.can she take multivitamins

Hi
Swelling of lymph node must be because of tubercular lymphadenitis.
It will resolve on treatment with Anti tubercular drugs.
The combination of Pantoprazole and Domperdone is available in doses of Pantoprazole 40 mg and Domperidone 20 mg, to be taken once a day.
She must have been prescribed Tablet Benadon (Pyridoxine) with ATT. In case not, ask your doctor immediately for that.
She can take Multivitamins.
For pain purpose, i would not advise any NSAIDs because they will aggravate the gastritis.
However in case of severe pain she can take a combination of Paracetamol and Tramadol.

One of the component of AKT is isoniazid, that causes peripheral neuropathy as it's side effect. That's why it is compulsory to give Pyridoxine with AKT which will help in preventing Peripheral Neuropathy.
