The region is his lower back. I just randomly addressed it as a wart. What could it be also the region around it got thick and hard

Not a wart, most probably a skin tag

Hi.

This is not a wart, and seems like an infected skin tag and will require a dermatologists review and examination for confirmation. I do not see why your dermatologist would not choose to excise it and send the tissue sample to a laboratory which is the best way to confirm the diagnosis.
