Suggest Remedy For Muscle Pain In A Person With Shingles After Taking A Pneumococcal Vaccine

Question: THIS IS FOR DR. SHOAIB XXXXXXX

THIS IS AN UPDATE FROM XXXXXXX FOR MOM WITH PAIN IN BONES AND BODY.

HER PRIMARY DOCTOR BELIEVES PAIN IS COMING FROM SHINGLES. SHE HAD SHINGLES LAST YEAR FOR SIX MONTHS ON THE RIGHT SIDE OF BODY. SHE HAS NO OPEN WOUNDS OR SORES, JUST PAIN ON RIGHT SIDE AND A LUMP OVER HER RIGHT SHOULDER BLADE. THE LUMP WAS THERE LAST YEAR BUT HAS NOW REAPPEARED. LIDOCAINE PATCHES WERE USED LAST YEAR. THIS YEAR PARENTS ARE UNDER A DIFFERENT RX PLAN AND IT IS EXPENSIVE TO PURCHASE THE PATCHES!!! SO I FOUND AN OVER-THE-COUNTER TOPICAL ANALGESIC ALONG WITH AN OVER-THE-COUNTER PAIN RELIEVER. BOTH SEEMS TO BE WORKING TOGETHER AND SHE ACTUALLY HAS MOBILITY. IT HAS ONLY BEEN THREE DAYS, BUT WE ARE GRATEFUL FOR HER BEING ABLE TO HAVE SOME MOBILITY.

MOM BELIEVES WITHIN HERSELF THE PNEUMOCOCCAL VACCINATION TRIGGERED OFF THIS PAIN IN HER BODY AND WE BELIEVED YOU WHEN YOU SAID FLUARIX CAUSES SUCH SYMPTOMS WITHIN THE BODY.

PRAYERFULLY, SHE WILL NOT HAVE TO SUFFER SIX MONTHS AGAIN.
I MADE THE MISTAKE OF CLOSING OUT THE QUESTION NOT KNOWING I WOULD NOT HAVE ACCESS TO YOU. I STILL HAVE TIL THE 26TH OF THIS MONTH BEFORE MY SUBSCRIPTION EXPIRES.

PLEASE LET ME KNOW WHAT YOU THINK DR. XXXXXXX I VALUE YOUR OPINION.

I OOK FORWARD TO HEARING FROM YOU AND I THANK YOU SOOOOO MUCH!

Brief Answer:
Keep using the topical and oral OTC pain relieving medications !!!

Detailed Answer:
Welcome DEE!

Thanks for consulting and your trust in HCM means a lot to me as a Physician. Your query had been there unattended for many hours. I have taken the advantage to help you as soon as I was permitted by the system to take the query. I have completely reviewed your mother's health issues, and well realize all those concerns. Being her physician, I will take care all of them today and will extend my expertise to answer the doubts.

Your mom is in pain and I feel really sorry for that. May God bless her with good health and happiness. Please extend my wishes "Happy Holidays" to her.

Her pain has multiple causative factors including shingles and then the vaccination shot she had. Actually she has a low immunity that is natural for this age group and she is having the pain triggered right after the vaccination shot. So I believe more in that she is having this pain more from the side effect of shot. Ingredients in her vaccination that are more likely to be the cause is Fluarix.

Yes shingles may also be one of the causative reason for her pain and an awful situation. But thank God she has mobility and is doing good with all of that. This is really appreciable that she is brave enough to cope with the situation.

I suggest to give her oral pain killers and apply topical ones too (as you are already doing and are on the right lines.. I appreciate that you found and alternative when your parents had a different health plan and Lidocaine patches were unavailable).

I want to add that vitamin D deficiency (although she has not massively low values) also contribute to bone and muscle pains. I suggest you to take her outside for 15 minutes in daylight for better production of vitamin D in her body. It will cause a very positive impact on her body. But remember to have applied Sunscreen lotion and wearing sunglasses (both you and your mom).

This time shingles, I hope will not make troubles for her for 6 months like last year. This time it definitely will go away soon. If she had an open sore then it must have taken much longer to heal up and for the pain to go away.

Please keep her diet balanced and healthy and give her plenty of water to drink. Fruit juices, vegetable diet and salads are good for her. For better digestion give her CITRUCEL DIETARY FIBER SUPPLEMENT easily available from any supermarket like Target.

Hope I answered all of your queries. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you. Please don't forget to give 'Five star rating' (if agreed) to help me assist patients in a better way; and, please don't forget to click on 'Close This Discussion'.

HELLO DR. HANIF

I DON'T KNOW IF MY ANSWERS GOT BACK TO YOU. I WROTE, BUT WAS INTERRUPTED. ONCE I RETURNED , I RESPONDED TO YOUR ANSWERS AND WAS VERY PLEASED. I HIT THE SUBMIT BUTTON BUT IT DID NOT SHOW IT HAD PROCESSED TO GET TO YOU, SO I WILL TYPE BRIEFLY AGAIN.

THANK YOU FOR TAKING MY QUESTION OUT OF THE QUERY.

THANK YOU FOR CONFIRMING ABOUT WHERE MOM'S PAIN IS COMING FROM AND THE NATURAL ALTERNATIVES WE ARE USING TO HELP HER. MOST COMFORTING. SHE IS FUNCTIONING AND WE ARE GRATEFUL!

MY MOM LOVES SALADS AND VEGGIES! SHE IS NOT MUCH OF A MEAT EATER. IF SHE EATS 3 BAKED CHICKEN WINGS, CARBO AND RICE, SHE IS FULL. SHE WILL EAT FRESH FRUIT WHEN THE PRICE IS RIGHT AND IF NOT HALF A CAN OF FRUIT IN ITS NATURAL JUICE OR NO SUGAR ADDED. SHE ALSO SNACKS ON HONEY ROASTED PEANUTS.

HER BREAKFAST CONSIST OF: GRITS, A BREAFAST MEAT AND 1 SLICE OF TOAST WITH JELLY. SHE BARELY EATS BREAKFAST. SHE HAS NO DESIRE TO EAT LUNCH. SHE LOVES DINNER. DINNER CONSIST OF: SOMETHING BAKED, VEGETABLE (S), AND A CARBO. SHE DOESN'T STICK TO THE AMOUNT OF FOOD DIABETICS ARE SUPPOSE TO EAT. SHE STATES SHE CAN NOT EAT THAT MUCH! SHE DOES DRINK WATER, BUT NOT ENOUGH.

SHE HAS A LARGE ROUND AND PROTRUDING BELLY. SHE IS 5'4" AND HER WT HAS RISEN TO 215LBS. SHE TAKES 2 METFORMIN ER 500MG AT BEDTIME. THIS GIVES HER GOOD READINGS IN THE MORNING WITH NO SIDE EFFECTS OR WT GAIN. SHE ONCE WAS GIVEN THEM TO TAKE IN THE MORNING, BUT IT GAVE HER BAD DIARRHEA. SHE WAS GIVEN GLIMEPIRIDE 2MG FOR DAYTIME, BUT THAT HAS CAUSE HER TO GAIN 7 LBS IN ONE WEEK. SHE STATED HER CLOTHES WERE GETTING TIGHTER ON HER.

COULD YOU PLEASE ADVISE ON WHAT DIABETIC MED SHE CAN TAKE FOR DAYTIME AND WILL NOT GIVE HER DIARRHEA.

I VALUE YOUR SUGGESTIONS AND YOUR CREDENTIALS ARE EXCELLENT TOO!!!!!

THANK YOU SO MUCH AND I LOOK FORWARD FROM HEARING FROM YOU.

Brief Answer:
Insulin therapy can be discussed with her treating Physician !!!

Detailed Answer:
Thanks for a follow up dear DEE!!!
I am happy to hear from you that I was of some help today for the health issues your Mom has been facing. I am pleased to hear she is functioning normally and this really is comforting. I extend my best wishes for the best of your Mom's health so that she may enjoy the holidays well as she used to do always in her life.

Well, I feel comfortable with the diet plans she has. There is not much a need to change them. Just add seafood and a lot of clean drinking water to make the best plan for a diabetic.
Belly fat is the worst thing in all humans but especially the mother of all illnesses in diabetics. It makes the peripheral INSULIN receptors more insensitive and creates more issues for diabetics to control the blood sugar levels. I suggest to start abdominal stretch exercises for better outcome.

Her blood sugar control as you stated is pretty much normal in range and under good control. So, I think she needs only the added benefits of some Physical activity rather than adding another oral antidiabetic in the morning. If she is having good glycemic control as she has, then there is less need for medication changes. If she has a poor blood sugar control and a deranged levels on blood test for Fasting blood sugar; and, HbA1c levels, then I recommend to shift her to Insulin rather than to add more oral drugs. All oral drugs have some side effects and she had drug induced diarrhea in recent past. She will have issues with other class of drugs too. But on her next visit she can definitely discuss the possible review about Insulin therapy by her treating Physician. In such circumstances this is the best approach and better side effects safety profile for diabetics compared to oral antidiabetic.

Hope I answered all of your queries. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you. Please give 'rating' (if agreed, five stars) to help me assist patients in a better way; and, please don't forget to click on 'Close This Discussion'.

HELLOOOO DR. HANIF

THANK YOU FOR YOUR ADVISE. THANK YOU FOR VERIFYING HER DIET IS O.K.
SHE REALLY IS NOT A MEAT EATER.

NOW, CONCERNING INSULIN THERAPY. SHE WAS ON VICTOZA. SHE WAS TOLD BY HER PCP THAT SHE HAS INSULIN, BUT IT NEEDS TO BE PULLED OUT OF THE PANCREAS, THIS IS WHERE VICTOZA CAME TO BEING.

MY FATHER'S RX PLAN WAS TERMINATED BY THE COMPANY HE RETIRED FROM DUE TO DEBT INCURRED FROM SO MANY RETIREES USING THE RX PLAN, SO I SEARCHED FOR ANOTHER. I FOUND A RX PLAN THAT FITS THEIR BUDGET ONLY TO FIND OUT VICTOZA WILL COST $786.00 A MONTH!!!! OUCH!!! SHE DID PETITION AND THE COMPANY SAID NO!!! AND THE SUBSTITUTES SHE TRIED SEVERAL YEARS BACK GAVE HER BAD SIDE EFFECTS. VICTOZA WAS A GOD SENT. SOOOOOOOO, SINCE THIS PAST JULY SHE HAS BEEN ON THE PILLS.

SHE STATES HER READINGS IN THE MORNING FROM TAKING 2 MEDFORMINS AT BEDTIME ARE WITHIN THE RANGE OF 89-93. SOMETIMES SHE MIGHT GET UP TO 126. WHAT IS A SAFE RANGE FOR MORNING READINGS?

THANK YOU FOR VERIFYING: SHE NEEDS TO GET INTO PHYSICAL ACTIVITY!!! SHE AGREES! NOW THAT SHE KNOWS A DOCTOR HAS SUGGESTED THAT IT WILL IMPROVE AND HELP HER LOSE THE BELLY FAT SHE IS WILLING TO GET ON THE TREADMILL THAT IS IN EXCELLENT CONDITION; IN THE BEDROOM ONLY COLLECTING DUST! I AM GOING TO ENCOURAGE HER TO GO TO PHYSICAL THERAPY WITH MY FATHER STARTING IN JANUARY SO SHE CAN STRENGTHEN THAT RIGHT KNEE AS WELL AS THE REST OF HER BODY AND HE CAN STRENGTHEN HIS MOBILITY AS WELL.

THANK YOU DR. HANIF. YOU TOO HAVE BEEN A GOOD SENT! I WILL LOOK FORWARD TO YOU GIVING ME AN ANSWER CONCERNING NORMAL READINGS FOR A MORNING BLOOD SUGAR LEVEL. THANK YOU.

Brief Answer:
Her blood sugar levels are already in the normal range !!!

Detailed Answer:
Hi DEE!
I appreciate your follow up query and happy to know your mom is doing well. Her diet is also what's needed for a diabetic patient.

All diabetics (Type II) have some insulin in their body which is manufactured by the pancreas. Type I diabetics have absolute Insulin deficiency. Type II mostly have obesity and an associated issue of unresponsive peripheral tissue Insulin receptors.

Resting muscles need Insulin to pull in Glucose from blood for their energy requirements. But exercising muscles are independent and self sufficient in that they don't need Insulin to pull in Glucose inside their cells for metabolism.

Her blood sugar levels in the morning are pretty much in the ideal range. Sometimes variations are tolerable if HbA1c levels every 4 monthly is in normal limits. You asked two times in your query so I want to assure that blood sugar levels of 100 mg/dL or below is perfectly fine for her and in my view with this level of fasting blood sugar she just needs exercise to stay healthy (no need to add medications).

I am happy, she agreed to start treadmill. She will be my ideal patient if she really starts physical activity like this.

Hope I answered all of your concerns. Let me know if you have further questions. I will be more than happy to clear any doubts. Please give your 'rating' (if agreed, Five Stars) to help assist patients in a better way; and, don't forget to click on 'Close The Discussion'.

HI DR. HANIF.

PLEASE ACCEPT MY APOLOGIES FOR YOU DID TEACH ME ABOUT EXERCISE HAS NO RELATIONSHIP WITH INSULIN PULLING GLUCOSE INTO THE CELLS FOR METABOLISM. I GOT IT NOW!

THANK YOU FOR CONFIRMING SHE DOESN'T NEED MEDS IN THE MORNING FOR HER LEVELS ARE IN NORMAL RANGE. AS FAR AS PHYSICAL EXERCISE... I AGREE, WE WILL SEE! SHE IS ENCOURAGED NOW, BUT I WILL PURSUE THE ISSUE WHEN MY FATHER STARTS HIS PHYSICAL THERAPY IN JANUARY.

BY ALL MEAN WILL I GIVE YOU A FIVE STAR.

BEST WISHES TO YOU, YOUR FAMILY AND YOUR PRACTICE.

HAVE A GREAT DAY!
