Suggest Remedy For Fatigue, Sleeplessness And Leg Cramps While On Diovan And Metoprolol

Question: Mom is 91...5 feet tall.. 113 lbs...she has htn and it runs 140ish over 47-53. She has diastolic htn, so the doc says. She has been diagnosed with afib( controlled), CHF (controlled.) she takes metoprolol 100mg... 60 mg nifedipine... 160 diovan... 25 mg hctz.... . She has been having problems sleeping though the night solidly. Loss of energy and get up and go.she gets leg cramps too. She heard about melatonin.
1. Any contraindication of mel with her bp meds?
2. Any particular bp meds that she takes that would cause such lack of energy?
3. Does melatonin start working soon sfter starting to take if? Thanks

You should know that melatonin is a regulator of the physiological circadiac cycle, and it may need some time (up to some weeks) to show its effects in regulating day-night sleep cycles.

From the other hand it is important to perform a differential diagnosis of other possible causes of this symptomatology ( chronic anemia, electrolyte imbalance, thyroid dysfunction, kidney dysfunction, heart failure, liver dysfunction, low blood pressure, etc) before concluding that it is related to her actual therapy.

What are her actual blood pressure and herat rate values?

I recommend consulting with her attending physician for a careful physical examination and some tests:

-resting ECG and cardiac ultrasound to examine her cardiac function
-chest X ray study
-blood lab tests (complete blooc count, blood electrolytes, kidney and liver function tests, thyroid hormone levels, fasting glucose, cortisol plasma levels, PCR, sedimentation rate for inflammation,etc.)

It is important to monitor her blood pressure during the day.

Metoprolol could cause sleep disorders and a low physical performance.

You should discuss with her doctor on the above issues.

Dr, her bp usually is 140-150 over 45-55. She has been described as having " diastolic" high blood pressure. A stiff heart. Im wondering about the metoprolol. She was taking just 50 mg of meto for a ling time but recently is taking 50 two times a day. She has one kidney

Her diastolic blood pressure, but the difference between systolic blood pressure and diastolic blood pressure (also called pulse pressure) is high.

This is a common finding in elderly patients, but also in aortic regurgitation and thyroid dysfunction.

It is important to optimise her CHF therapy (diuretics and vasodilators).

Regarding Metoprolol, besides the fact that she has only one kidney, there is nothing to worry about as long as her renal function tests are normal.
