Suggest Remedies For Fatigue While On Losartan And Coreg

Question: I just got on bp meds about 1mos ago (coreg 40 and losartan 50/hctz 12.5). I take losartan in around noon and coreg ext. release at night. I have been walking 15 mins (2x's) day as per my dr. (cardiologist). I have LVH with over 55EF and working normally. If I try to do too much I feel tired. I am having to eat every 2 hours and plenty of protein or else I get weak and tired. What should I do?I forgot to mention that my weight is around 153lbs. My bp is staying down but then it goes up when I try to do something that exerts a small amount of energy like using my arms to like lift a lightweight pkg. (2 lbs approx.) and open it. Then my bp goes up if I haven't eaten enough protein and then my arms feel tired. I have been eating protein foods all morning a banana and a piece of toast to get my energy back up so that I can go walk in a minute also drinking lots of water.

I have gone through your medical history carefully and understood your concerns.

1. Feeling weak with no energy requires further evaluations:
- did you have this feeling before you start the bp meds?
- or you had this feeling after starting the bp meds?
- have you also checked your blood sugar levels?
- what about your liver/kidneys function? are they normal?
- have you got your electrolytes checked also?


2. As I was reviewing your meds, I noted that you are taking HCTZ together with losartan. HCTZ is a diuretic, and its use is associated with loss of electrolytes. This imbalance is associated with feeling weak as in your case.

3. Oscillations in bp and blood sugar levels can also give the same symptoms you are having now, That's why I always recommend my patients to closely monitor them.

4. A good cardiac check up is needed to evaluate the status of your blood vessels (especially of thoracic and abdominal aorta as they are the largest vessels supplying the internal organs with blood).

my blood sugar was 120 non fasting prior to taking losartan. all of my blood work (lipids) are in normal ranges ldl is 133 non fasting, total chol is 206, tri is 94 and hdl 54....all non fasting. It doesn't appear that I have any liver or kidney problems as per my blood work (I can send blood work if you need but it's all in normal ranges). Thyroid is minimally over at .05 and both family dr's have said to do nothing. Just had electrolytes blood test done and I have a 4 for potassium as per my cardio dr. and electrolytes look good. He did an echo and 1.9 muscle thickness and treadmill with echo and no obstruction. As far as I know everything has checked out and the dr is not concerned about anything....all I have is pvc's and pac's (wore a holter for a week).

I think you are on to something....I am starting to think its the losartan/htcz? I don't think I drank as much water as needed yesterday...maybe that's part of this. Please let me know if you think I need further evaluation. Thanks so very much. XXXXXXX Dunlap

Thank you for providing the requested information.

I am glad to hear that all the blood tests and cardiac check ups you did resulted fine. This puts my mind at peace for all the other pathologies.

Taking all these into account, I admit that all your weakness is due to diuretic as it is inducing frequent urination that might make you feel thirsty.

As the electrolytes are fine, then, everything is related to loss of fluids. Increase water intake for a couple of weeks and see the changes. Do not forget to have your meals regularly, do not skip them.

After 2 weeks of increased water intake, depending on the changes, can discuss again.
