Dear Sir
The following discussion pertains to esophageal spasm in general.
1. The spasm may be induced on its own i.e. de-novo or it may be secondary to gastroesophageal reflux disorder (GERD). It is felt as a sharp pain or sometime as a spasmodic pain or a void in the chest.
2. Repeatedly sipping lukewarm water in small amounts helps in relief of spasm.
3. If it occurs secondary to GERD, certain measures are helpful in its prevention. This include not taking fluids/ cold drinks and water with you meals. Taking meals well before your bedtime (at least 2 hours). Avoiding citrus fruits with meals. Keeping head end of your bed higher than the foot end.
4. Antacids (Magaldrate, sodabicarb etc), proton pump inhibitors (like esomeprazole, omeprazole or pantoprazole) or H2 receptor antagonist (ranitidine, cimetidine) all are mainly helpful in patients where esophageal spasm is associated with GERD.
5. Esophageal spasm may be more common in patients with GAD (generalized anxiety disorder). A psychiatrist or clinical psychologist will help best in that case.
6. Esophageal spasm without GERD is generally a motor disorder of smooth muscles and requires detailed evaluation by gastroenterologist.
7. Esophageal spasm can be related some more serious disorders and should be evaluated and managed by gastroenterologist only. Any kind of self medication may be harmful and not advisable.
