Question: Dear Sir ,

Request you to give me the best available multivitamin tablet with minerals for my overall general well being .Iam male aged 45 years and diabetic but under control .Iam over weight 90 kgs height 5 ft 8 inch .Iam on a portion controlled diet but would want the pills for my vitamin and mineral requirements ,skin ,hair ,eyes ,bones etc .I would preferably require a XXXXXXX medicine .

Madam ,

I wish to reduce my weight and it has not reduced since last 3 years .I have for the last 6 months started diet and exercise but have not reduced my weight a bit ,It is stagnant at 91 kg .I was advised to take orlistat 120 mg .Wanted to know if it can be taken and if it can be taken in conjunction with diabetes control pills iam taking daily ie glycomet gp 2, roseday 20 mg and PPG Met 0.3 .Or if you advise against it please advise if any alternative medicine can be taken as bulk of the fat is located in my abdomen region .

There is not adverse interaction of orlistat with your diabetes drugs, but the thing is that orlistat hampers the absorption of fat from the intestine. And being a diabetic you must be already on a restricted carbohydrate diet and your diabetic drugs hampers the absorption of carbohydrates from the intestine.

Now, as the principal energy source for our body are carbohydrates and fats, so taking the two together will cause weakness and will increase the possibility of you going into hypogylcaemia.

So you need to start orlistat at a very low dose and then gradually increase it as you can tolerate it. Start with half tablet at lunch time, then make half at dinner too, then full tablet at lunch and half at dinner and finally full at both lunch and dinner. Then you may take it at breakfast also. But be slow in this ascent. And always keep a small chocolate or any glucose biscuit in your pocket, as a precaution for hypoglycemia.

This if you can achieve will greatly benefit you, not only in reducing your weight, but even may lead to reduction in the dose of your diabetic drugs.
