Suggest Medication For Fever And Respiratory Infection To Replace Amox And Levolin

Question: My son is 5 yr old and he has fever 101 + °F since yesterday. My doctor has diagnosed upper respiratory infection and suggested amox, Levolin and call for fever, however after giving the anitbiotic my son vomits. Now I have given call and the fever hasn't come down. Currently I am sponging to get the fever down. Any suggestion please

I have gone through the query and do understand your concern.
During upper respiratory infection, there may be fever of high grade during initial 72-96 hours. Giving paracetamol in proper dosage that is 10-15 mg/kg/dose along with tepid sponging helps in most. You can repeat the dose at an interval of 4-6 hours depending on the dose which you are giving. If you give 10 mg/kg/dose it may be given at an interval of 4 hours while with 15 mg/kg/dose it should be repeated after 6 hours(the total dose should not exceed 60 mg/kg/day).
Continue tepid sponging of whole body from head to toe so that a thin film of water forms that evaporates till you reaches the lower part of body and thus leading to release of heat from body. Continue this at least for 15-20 minutes.
If fever persists even after this you may need to consult your pediatrician for prescription of mefenemic acid (decreases fever with different mechanism and is more effective). The dose is 6-8 mg/kg/dose(maximum 25 mg/kg/day) can be repeated at an interval of 8 hours.
If the child has vomited the antibiotic syrup within 10 minutes of intake the dose should be repeated.Antibiotic helps in controlling the infection and does not helps in immediate relief of fever.
Vomiting can be there due to fever so better to give antibiotic once fever subsides.
