Suggest Medication For Anxiety And Panic Attacks Apart From Rivotril

Question: Dear Doctor. This is further to our discussion.
1) For depression i am taking venlift od 150 mg one capsule a day.
2) For social phobia means unable to express facial feelings (facial freezing) i am taking indertal 160 mg per day.
3) Of course for general anxiety and panic attacks I am taking rivotril 2mg. But doctor there is no improvement in this. Means when there is a group of people, when the turn is mine, I am unable to explain because of panic attacks. Also when there is an argument situation the same thing palpitation occurs.
4) Dr. depression and unable to express facial feelings improved by 90%.
5) Kindly help me in the panic attack situations as I am a Exeutive in the Central Government and lot of interaction and group discusion with people, kindly help me which medicine exactly helps me to talk freely without palpitations. As I have good posture, knowledge and subject but I am unable to kindly help me in this.
6) Also in the process of argumentable situation ireespective of gender and age I cannot argue with the people. Kindly give me any exact medicine so that I can overcome this problem.

Thanks for contacting the Ayurveda section of [A]YUSH and after reviewing your case and medications, I'm of the following opinion:

1. Since you are already being supervised by other learned physicians on this portal you should consult with your healthcare professional for CBT [cognitive behavioral therapy] to identify and counteract negative thoughts.

PS. As they have given due consideration in letting you know about the modern parameters, I will limit myself to the Ayurveda physiology

2. Because panic attacks are repeatable [argument situations] which means you feel endangered and unable to escape which means there is ‘fear of fear’ i.e. vicious anxiety- panic attack- anxiety cycle, thus before taking panic attacks to be due to anxiety or depression make sure you have ruled out following medical conditions like overactive thyroid, low blood glucose, use of excess caffeine, cocaine, amphetamines or heart problem.
PS. practice relaxation techniques like yoga, meditation but also Pranayam which will strengthen the relaxation response of body as opposite to stress response during panic attacks, i.e. don’t try to flee away from the situation which will reinforce the perception that your panic attacks are unbearable [remember they are uncomfortable but not life threatening], allow the symptoms to pass and you will gain ability to cope with them

PS. Practice stretching Yoga poses [Dhanurasana, Matsyasana, Janu Shirsasana, Paschimottanasana, Hastapadasana], hot shower and massage are simple techniques to overcome muscle tension from anxiety.

3. Since the said problems can be grouped under GAD [generalized Anxiety disorder], Dietary Regime and Lifestyle management along with Medicine is initiated from my side according to “manovaha srotas vyadhi”

4. DIETARY REGIME

i. Avoid alcohol, pepper, garam masala, mustard oil, pickles, rye, spicy food, anger, grief, fear, late night waking, garlic, ginger, onion.

ii. Take: mint, coriander, tomato, and cumin, plenty of water, vitamins and mineral supplements, apple, banana, grapes, dates

5. HOME & NATURAL REMEDIES

- Lemon juice mixed with sugar in a glass of water helps release stress removing hormones from the body and bring mind at peace/rest OR eat 2 spoons coriander [Dhaniya] seeds with 2 spoons of sugar candy [mishri]

6. Apple, almond, banana, broccoli, cantaloupe, melon, carrot, cashew nut, corn, cauliflower, grapes, guava, lemon, XXXXXXX orange, pineapple, soybean, spinach have antioxidant and detoxifying properties so take them in plenty.

1. Venlift [venlafaxine] is used in the treatment of depression and symptoms associated with major depressive disorder, and anxiety, aggressive behavior are noticeable ADR [adverse drug reaction] of the drug and even if your doctor asks you to stop the drug, you should not discontinue it abruptly, rather a gradual tapering of dose is recommended

PS. check with cholesterol level and liver function tests [LFT]

2. Inderal [propanolol]: because of its beneficial effects on the heart it is helpful for people with ‘Social Anxiety Disorder’/Situational Anxiety: where public speaking or performance is involved

PS. It helps mainly with relieving physical symptoms [performance/situational anxiety] and not mental anxiety.

3. Rivotril [clonazepam] is mainly advocated in panic disorder and since coordination disturbances, problem with thinking and memory is the commonest ADR of the drug thus could explain the same in you

PS. you may ask for combination of clonazepam and propranolol [if they are to be continued]

4. In all the three situation(s), your attending physician/doctor is in the best position to help you make a decision on what to do.

5. Medicine for panic attacks in Ayurveda are tailor made and not given according to the market, so I can only help you with the medicine which I prepare myself.

1.     After going through your symptoms and the main Dosha [aggravated Prana vayu] which weakness the nervous system and triggers mental imbalance associated with it, I’ve come up with the following formulation which will increase the 'sattva guna':

i. Oil for de-stressing helps reduce muscle tension and stiffness and thereby toning and rejuvenating [360/- for 200 ml]

ii. Medicine to improve metabolism, reduce toxins and help eliminate toxins, thereby building body’s resistance. [685/- for 60 capsules]
