Skin And Hair Is Very Oily. Having Hair Fall And Some White Substance At The Root. Suggest Further Treatment?

Question: Hi,

I am a 20 year old male student. My hair mostly , and skin have become very oily recently. I have a shower my hair is normal and without any oil and then after about 8 hours it becomes oily again. This is not the major problem though. The main problem is that my hair has started to fall a lot. The hair strands fall out with some white thing at the root. Can you help me please? Should I do any blood test for any lack of vitamin or any other medication. Also can you advice me on some good hair routine and products. And also maybe a diet for my problem.

I can understand the whole series of problems related to hair. As a young man you are quite worried about all the things related to your scalp.

Now taking all the problems one by one:

* FOR OILY SCALP AND HAIR ROUTINE: It is quite usual to find oily scalp in your age group. We have maximum number oil glands concentrated over face and scalp. Our oil secretion is dependent upon the hormonal level. During twenties of our life, the hormones especially the male hormone testosterone is at peak resulting in high oil secretion. This also depends upon weather condition and consumption of diet. If the weather is hot and humid there is always a tendency for high oil secretion.
The same is true for consumption of spicy food items which stimulate oil secretion. But most of the times it is simple hormonal thing.

For the remedy part follow the things below:

1.Wash your scalp daily. I guide my patients to wash twice in a single bath. It means when you are washing your scalp apply first time shampoo for washing the hair part only, then rinse it off immediately with water. Reapply the shampoo again immediately, leave it for around 5 minutes, do a good gentle massage by your fingers for around 2 minutes and then wash it off. By doing so for around 7 days you will get good benefit.

2. Stick to one good shampoo which is suiting your hair, don’t change the shampoos frequently. If you are using any conditioner also then to be used over the strands of hair only and not over the scalp skin.

3.Do not use any kind of hair oil or any oily or creamy stuff any time on your scalp.
4.Comb your hair at least 3-4 times a day and most importantly at night time so that the dust particle and other of dead skin is taken off.So for betterment of high oil secretion the only good solution is daily washing with any good shampoo. Reduce your exposure to hot and humid climate with any consumption of spicy food.



* FOR THE PART OF HAIR FALL: It is quite common to find air fall in men which is known as "male pattern baldness" is most common cause of hair fall in young men like you.

This process of falling starts by 18-20 years and is highest in between the age of 20 to 30 years. I can see you are a young man of 20 years. So it is quite obvious that you can have the starting of this problem of hair fall just because of hormonal influence.

Other than male pattern baldness there can be falling of hair all over the scalp can occur after simple stress too !!!.

Generally there are no investigations required but many a time we see low hemoglobin level and serum ferritin level resulting in hair fall. So if at all you want to go for an investigation then go for Hemoglobin level and serum Ferritin level. If these are less then can go for iron supplementation in addition to good diet.

Nevertheless in hair fall poor diet contribute to increase the hair fall. SO you should eat healthiest of diet.

For the betterment of diet kindly follow the suggestions below:

1. Eat at least 3-4 different fruits saplings per day. Every fruit is good but you can make a combination of sour with sweet fruits such as oranges, papaya and other of seasonal fruits.
2. Pulses are good source of proteins. So everyday have a small bowel full of mixed pulses. Add green leafy vegetables additionally in the pulses which contains lot of iron and vitamins in it.
3. Sprouts are the best source of fibers, minerals especially iron, vitamins and proteins too. so add it at-least alternate day if you can not make in every-day's part of meal.
4. Over night soaked and peeled almonds will give you good vitamin E which is good for the hair growth.
5. Milk and milk products should be included in every-day's schedule.
6.do mild exercises, reduce your work stress as all these have bit impact on skin so also on scalp skin.

You can follow the above regimen for 8-12 weeks periods. Finally if you are not getting relief from all these measures in this time period then can apply and take certain medicines under medical guidance. But this is my personal suggestion as a dermatologist that rather than medicines, simple daily regimen works better than anything.

Thank you for your answer. I just wanted to know a bit more for my case. First of all my daily hair fall is about 30 - 60 hair strands and it happens all over my head so i don't think is male pattern baldness. At the moment i am going through a stressful period due to changing places and university. Which vitamins are mostly essential for good hair? Can you please recommend me any good hair shampoo for oily hair and one for dandruff ( even if expensive) because i am not very good at understanding them. What about Biotin , is it good supplement to take? ( another doctor recommended this) What about the hemoglobin level , Ferritin level and iron level, can this be checked by blood test and how can they be normalized. I would be very grateful if you could help me with this information.

Regarding hair fall of 30-60 hair- Dermatologically or in terms of medical science we consider it normal if the hair fall is below 100 hairs. So in your case if it is 30-60 hair each day then it is quite normal of the thing. It is common fact that we see what it is falling but we cannot see the one which is growing. So each old hair is replaced by new hair and that is why we see replacement of old hair as falling hair.

Regarding male pattern baldness or simple hair fall- In men it is combination of both the process. Majorly it is hormonal meaning male pattern baldness and also influenced by stress and nutrition too. So it is not a thing of worry for you. Just reduce your stress level and eat healthiest of food as suggested before.
The most important of vitamin for hair growth is Biotin which is part of doctor to take Biotin supplement. You can very well start the same. Take it atleast for 3 months for good results.

Regarding shampoos- for regular wash you can take Laureal or Tresemme. These are good shampoos. For dandruff any medicated shampoo with Ketoconazole 2 % in it is the best.

For the part of hemoglobin level , Ferritin level and iron level- Simple blood tests are sufficient to know the level of hemoglobin level and Ferritin level. Ferritin level and iron level are one and same thing so ferritin level will tell you best regarding the same. If these are low then you can start Iron tablet along with biotin.

Thank you for your help. One last thing i wanted to know is if the hair density and strength can be improved in someway. And which haircut is best for for oily hair ; short, medium or long. And can oily hair become normal or should i keep washing everyday for the rest of my life.

Regarding your further queries: You can take biotin as the best supplement for the hair and additionally can take iron supplementations told before.

For the part of hair cut, short to medium hair cut done once in 3 weeks id best. The skin changes its secretive tendency after every 5 to 10 years. So after the age of 25 years you will be having normal, kind of scalp. So XXXXXXX not worry about the same.

For the further query of itching on the scalp- Kindly follow the instructions below:

- Wash your scalp daily with good anti-fungal shampoo containing Ketoconazole 2% lotion.

- Apply lotion containing mild steroid like Betamethasone Dipropionate lotion or Beclomethasone Dipropionate or Fluocinolone lotion once at night time. DO not use beyond 14 days.

- You can take antihistamine like loratadine tablet (claritin) for two times a day for 7 days and once for next 7 days.

- Do not scratch or touch the area as it can cause further aggravation of the area.
