Is Switching From Sulpitac To Bupropion Safe?

Question: Good evening doctor - went for s second opinion yesterday. The doctor made following changes :
Quetiapine sr 100 - same as earlier
Sizopin 25 - same as earlier
Peragabid asked me to stop
Tynept 12.5 made thrice instead of twice
Sulpitac 100 made half for 8 days and then stop
Added bupron xl 150 - half for four days, then 1 next four days and then 1-1 mor/eve
Inderal la 20 same as earlier
Zapiz 1/2 of 0.25 in the night - same as earlier
I have following queries :
Is switching to bupron from sulpitac is a good idea ( will it work better than sulpitac ), with sulpitac am doing more or less ok
Is it better to take tynept od instead of regular and in the mor or eve and what is the half life of tynept od
Will stopping sulpitac have any withdrawal symptoms in terms of anxiety
Pls advice.

You can continue to take low dose Sulpitac.

Hi, thanks for asking again

- Sulpitac and Bupropion are two different drugs, the former is an antipsychotic while the later is antidepressant. But both have some dopaminergic actions. Addition of Bupropion would help to augment the action of Tianept but in my opinion the drug will not help much in anxiety. If I have to choose one out of these two drugs considering the history of symptoms then I'd prefer Sulpitac in low dose.

- Yes you can take Tianept OD instead regular. This will be more convenient and the formulation results in sustained actions.

- Stopping Sulpitac will not cause withdrawal in terms of anxiety. The drug can be stopped without any worries after taking 50 mg dose for a week. You can even stop the drug cold turkey.

- Since with Sulpitac you were not feeling 100% better so you can give Bupropion a try, but don't expect much wonder with the drug.

Hello again,

- Better to take the medicine after breakfast in morning.

- The half life of Tianeptin is about 3-6 hours, but since Tynept OD is a once day formulation the tablet results in sustained delivery of drug through out the day. I am not aware of exact half life of Tynept OD 37.5 mg tablet but the company claims its effects last for about 24 hours.

- Bupropion is not a SSRI drug. The drug improves adrenergic, dopaminergic action by direct effects and serotonergic actions by indirect effects. The drug is commonly used as add on drug for depressive disorders.

The drug should be taken at time of meals. Though intake after meals affects its absorption but the maximal effect achieved remains same irrespective to whether the drug was taken empty stomach or after meals.

I prefer to prescribed the drug after meals.

Hello again

Both Bupropion and Sulpitac are safe and don't cause gastric disturbance. Both are dopaminergic drugs and thus are known to reduce gastric acid secretion.

Even Sulpitac or Amisulpride is used in Irritable Bowel Syndrome by some doctors.

Abdominal pain can occur rarely with Bupropion but that is seen in high doses.
