Is Serum Creatinine Of 125 A Cause For Concern?

Question: Good evening,
My history is as follows: Childhood Wilms Tumour, Total neprectomy of left kidney, partial right neprectomy, Gestational hypertension, CKD3 and Diet controlled Diabetes.

I had one and half kidneys removed before I was three. With three of our four children, No. 1, 3 & 4, I had problems with gestational hypertension. I was diagnosed as Type 2 Diabetes in January of this year.

I am of European descent but was born in Zimbabwe, XXXXXXX I now live in UK and am 39 yrs old, 40 in November. These are my recent blood tests results.

Sodium 139 mmol/l White Cell Count 7.2 x10^9/1
Potassium 5.7 mmol/l Red Blood Count 3.64 x 10^12/1
Urea 12.0 mmol/l Haemoglobin 106.0 g/l
Creatinine 126 umol/l Haemotacrit 0.3 %
eGFR 46 ml/min MCV 85 fl
Albumin 39.0 g/l MCH 29.0 pg
Adj Calcium 2.66 umol/l Platelet Count 382 x10^9/1
Phosphate 1.27 mmol/l
Alk Phosphate 99 mmol/l
Cholesterol 2.90 mmol/l
Parathyroid Hormone 1.5 pmol/l
Ferritin 29 ug/L

The Dr said my renal function has remained stable and the most recent serum Creatinine was 125.
Please can you tell me which should be areas for concern and which areas I am ok?
I do not drink or smoke, and never have. I weigh 75kg at present, having lost 5kg in the last 6 months.

Thanks for the query

I understand your concern regarding renal injury. Your creatinine is fair and you do need not worry about it right now. However here is what I suggest

1. You will need to lose more weight, I recommend you to maintain your body mass index at 20
2. Make sure you do not consume red meat. Chicken and fish are ok.
3. Reduce your salt intake
4. Get a urine routine test done next time and see if you are losing proteins
5. Single most important entity is to make sure that your diabetes and hypertension are under control
6. It is proven beyond doubt that ACE/ARB tablets help in long term renal protection, you must be on it. Approach your doctor with my suggestion
7. Your potassium is little on the higher side. Please avoid a high potassium diet like fruits and juices

Your rest of the parameters are ok, I recommend your Hemoglobin should be around 11.6, iron supplementation will help improve it.

Thank you for your reply.

1. Is the Sodium level too high? And if it is, what is a 'normal' or acceptable level?
2. At the last examination, they said I was not losing any protein.
3. My diabetes reading today was 7.4.
4. At the last few examinations the Dr's said my BP was on the slightly low side.
5. What exactly are ACE/ARB tablets? What would they be called in England?
6. The Dr said my potassium levels are mostly fine, he thought it was possibly just a blip on that test.

I have been keeping to a low carb diet and this is what seems to be helping with weight loss and has helped to reduce my sugar levels.

Thanks for getting back. Here is the reply to your doubts

1. Sodium of 135 to 146 is considered normal however there will be some changes based on lab differences
2. arb is angiotensin receptor blocker and ace is angiotensin converting enzyme blocker. These tablets are known to cause enzymatic blockade which leads to protection of both heart and kidneys. They are known as ace/arb even in England. I usually start telma 20 in patients with lower blood pressure
3: I am happy you are losing weight , please continue to do what you are doing
