I am taking Telma H 40 and Aten 50 in the morning and Telma 40 in the evening.
I have urinary tract blockage and going through dilatation twice in a month in that period I took antibiotic tablets for five days. My creatinine is 0.9 and uric acid is 7.4 and cholesterol HDL is low rest reports are ok. Please guide me with detail.

Brief Answer:
Lifestyle modification essential

Detailed Answer:
Hello

Thanks for getting back, I am sorry for the short delay in the reply I just came back from the hospital.

You are taking the right medications for hypertension. Since you are on Telma H make sure your serum electrolytes are checked once in 2 months, it is known to cause low sodium and low potassium ( electrolyte disturbance)

Here are my suggestions to you

1. I recommend you to get a urine test done to rule out residual infection, most urine infections are treated for a period of 7 days. I would also like to get your residual urine scan done to see of your bladder is able to completely void the urine without any residue
2. Your creatinine is normal
3 Uric acid is high, it will require treatment. I recommend you to take Tab Fabuxostat once a day. Uric acid is high in people who take a lot of animal meat, please cut down on animal meat including fish to just once a week. Also cut down on alcohol and food rich in fat.
4. Low HDL is because of lack of exercise, I suggest you to exercise everyday for atleast 30 min. The best exercises to increase HDL are swimming, jogging and cycling. Tab Atorvastatin once a day will also help to increase HDL

I hope I was of help, if you have any further queries please get back to me

regards

Brief Answer:
Urine routine and Ultrasound with post void residue has to be done

Detailed Answer:
Hello

KFT will only tell you about the level of kidney function and electrolytes. What is more important is to know if there is going to be any residual urine in the bladder. So please get an ultra sound scan done to rule out any residual urine in the bladder after voiding urine. Urine is the bladder can cause repeated infections and cause harm to the kidneys

Viralex is taken once daily, if you are taking Viralex then no need to take Shilajit. I personally as a nephrologist request you to avoid natural remedies as some of them contain heavy metals which can harm the kidneys

Both drugs I have prescribed is to be taken for 3 months, you will have to recheck your lipid profile and uric acid to know if it has to be continued.
There is no harm in continuing atorvastatin for life long for better cholesterol control.

I hope I was of help, if you have any further queries please get back to me

My ultra sound scan was done recently before 10 days, the residual urine in the bladder was about 25%, "is it OK or any harm" ?. Please tell me virilex and shilajit have metals on it, I am taking Shwami Ramdev Shilajit twice a day. On my last question I asked about the dilatation, is it safe or not ? because first two was done weekly and third was after two week and next will be after one month every time I took antibiotic for 5 days. Is this harm my kidney? what I do to live long with one kidney? one more thing I have 20mm cyst on my kidney. Please suggest me.

Thanks

Brief Answer:
significant residual urine .

Detailed Answer:
Hello

Thanks for getting back

I am sorry for the short delay in my reply. It was a very busy day at the hospital .

Many people have single kidneys, in fact even our ex prime minister Mr atal bihari vajpayee has a single kidney. One kidney is enough to lead a normal long life.
The only precautions you have to take are

1. Do not swallow pain killers unless prescribed by your doctor
2. Mention about your single kidney status to your doctor before he prescribes so that only kidney safe drugs are given
3. keep yourself well hydrated . Drink 6 glasses of water per day
4. Do not smoke

You have significant post void residue, anything more than 10% is significant. It can lead to repeated urine infections. Please Appraoch a urologist soon

There is no harm in dilating, in fact it makes sure that you don't have residual urine .

I do not know if virilex has heavy metals in it. Most products donot undergo adequate testing before release to know for sure. If I were to be your doctor I would ask you to avoid these medications . I suggest the same to you, however speak to your doctor once before stopping

I hope I was of help, I am once again very sorry for the delay
