Question: My wife is having galblader stones.and having SGPT level 106.can we go for gslblader removal surgery.

Brief Answer: Medical intervention is enough, if asymptomatic. Detailed Answer: Hello there, Nice to meet you here, The normal level of SGPT [Alkaline transaminase] is 7 to 55 units per litre of serum, as any increase in its value directly related to the liver damage. They are mostly elevated by liver damage (caused by underlying insults, mostly blockade of common hepatic ducts) by residing gall bladder stones. As you are mentioned only elevated SGPT level (106), I would suggest only medical intervention [usage of bile salts], no need for surgical intervention , unless gallstone are symptomatic and causing emergency symptoms. In order to measure the seriousness of injury, I also suggest you to do other liver function test like SGOT, gamma glutamyl transpeptidase (GGT ) level to your wife. Consult your treating surgeon as soon as possible if her symptoms are worsening and get treated accordingly. Hope helped your query, Dr.surendiran.g

Thanks doctor for the reply. Actually my wife got sevier abdominal pan and doctor suggested to go for sonography and MRCT scan.and scan reports says that "multiple tiny subcentimeter size calculi are seen in the gall bladder".and doctor suggested for laparoscopic surgery for galblader removal. And subsequent he suggested liver function tests.and have get it done and the results sre SGOT-415, SGPOT-520, ALKALINE PHOSPHATE-206, total bilirubin-2.43, dirct bilirubin-1.60, indirect belirubin-0.83, total proteins-6.7, serum albumi-3.6, serum globulin-3.1.after these results doctor given antibiatics -MAGNEX FORTE 1.5 g.after this medication for one week all liver results are came normal , except SGPT-106.I request your suggestion can we go for galblader removal surgery with this SGPT range 106.or should we waitl till it reaches normal range of 40 IU/L Regards. Detailed Answer: Hello sir, Nice to see you again, As your treating surgeon provided with the medical treatment [MAGNEX FORTE] already and reduced the level of the liver enzymes [SGPT 106, reduces slowly], I strongly recommend you to go for ELECTIVE laparoscopic CHOLECYSTECTOMY [gall bladder removal], after routine investigations. Since there are multiple sub centimeter stones, it has to be removed. Otherwise it will bring back with severe complications, by obstructive the CYSTIC DUCT [gall bladder neck]. Hope helped you. Thanks again doctor. Yesterday again we have tested SGPT and level is 86 and it is reduced from 106 to 86.as you told its reducing slowly. And as you mentioned surgeon recommend for laparoscopic surgery (galblader removal).now we want to go for surgery on Monday. Kindly suggest can we go for surgery if SGPT level is at 86 IU/L or should wait some more time for surgery as it reaches to normal range (40IU/L). Regards. XXXXXXX

Detailed Answer: Hello sir, You are welcomed, Yes, she can have the laparoscopic surgery as per surgeon advice on Monday, since every enzyme level dropped to normal and as I said already, SGPT level alone is not of serious consent.
