Posted on Mon, 15 Dec 2014
Question: 2 hours after my dinner, my blood sugar is 134. I will go to bed shortly and am wondering if this is too low for overnight. Should I bring it up 10 or 15?

Brief Answer:
Diabetes and low glucose reactions

Detailed Answer:
In general, many diabetes experts recommend a bedtime blood glucose target of 110-150 mg/dl. So by that token you are in the desirable range. However, diabetes management and glucose goals are highly individualized. When I see someone like you in my practice, I set reasonably tight glucose targets such as the one alluded to above , provided none of the following are present :severe frequent low glucose reactions, lack of awareness of low glucose reactikns, short life expectancy, poor psychosocial support, lack of self motivation, presence of diabetes related complications (heart attacks, strokes, kidney failure etc). Essentially when I see a relatively young individual such as yourself and there are no major associated complications present then I recommend stringent glucose control to prevent diabetic complications and prolong life. Dosing of Lantus and novolog are challenging and the patient needs to be trained in learning how to adjust these doses based on several factors such as recent A1c test result, body weight and amount of carbohydrates consumed at meals.
