Brief Answer:
Prevention is better than cure.

Detailed Answer:
Hi.
Read and understood your concerns.
And very happy to know about your awareness, I fully agree with you for the thoughts you have put in.

- You have Shingles in the butt crack, drying up and on medication for 5 days. Your apprehension is absolutely right about the skin getting infected due to the constipation or possible soiling as feces is full of bacteria and you might have caused some skin cuts due to itching.
I would like to give you an antibiotic like Cefixime or Oflaxacin whichever suited you in the past and Metronidazole along with probiotic and a laxative. You may please need a prescription of a local Doctor.
*Prevention is better than cure.

Your second point is about the periods in 4 days. Periods cause congestion in the pelvis and if you do not suffer from PMS, nothing to worry please. Anyways Bed-rest is a part of treatment in Shingles. Take full bed rest.

Shingles will not grow due to periods. Nor you will catch any bacterial infection as you will be taking the medicines mentioned above.

Thanks for your thinking and vision.
Please feel free to ask if there is any gap-of-communication.
Wishing you the best possible recovery.
