Brief Answer:
They are prescribed by considering associated risks.

Detailed Answer:
Hi

Thank you for asking HCM

I have gone through your query. These 3 drugs have different advantages. It is selected by considering associated individual risks with hypertension.
Telma helps to give protection to heart and kidney from pathological modification along with lowering BP. In diabetes this is given in low dose even though there is no high blood pressure for kidney protection. But if dry cough is there then it can be changed to Cilacar.
Cilacar also helps in lower BP and gives protective effect for heart and kidney. Metalor is given when you have persistent palpitation or increased heart rate along with hypertension.
So if your heart rate is normal and no palpitation then I would recommend Cilacar.
If BP is high the combinations of these can be used also.

Let me know if anything not clear.
I am happy to help you.
Thanks.
