Is C Section Advisable When There Are Two Loops Around Baby's Neck?

default
Posted on Wed, 22 Oct 2014
Question: Hello

I m 36 weeks today n went for scan

Baby has two loops around his neck one loose and other closely applied
Can this cause any problem before the labour starts?

Second baby weight is 2.34 kg at 36 weeks

Can I go for elective c section ?
doctor
Answered by Dr. Dr. Rakhi Tayal (36 minutes later)
Brief Answer:
Elective C-section is preferred.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
At 36 weeks if there are two loops of cord around baby's neck and one of these is closely applied then the chances of loosening of the cord are less and elective cesarean section is preferred after 37 weeks.
You need to keep a count of your daily fetal movements till then. There should be at least 10 movements per day. But counting movements the entire day is not easy. So after every meal (breakfast, lunch and dinner) you can lie left lateral on bed, count your movements for one hour and mark them on a piece of paper without doing anything else in that one hour.
If in that one hour , you perceive 3 movements, they are adequate. So after every meal you should have at least 3 movements.
Ultrasound with BIOPHYSICAL SCORING of the baby is a must in decreased fetal movements.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.

By biophysical scoring
Do u mean CTG or ultrasound ?
doctor
Answered by Dr. Dr. Rakhi Tayal (1 hour later)
Brief Answer:
It needs both.

Detailed Answer:
Hello.
Thanks for writing again.
Biophysical scoring means both CTG- for non- stress test and an Ultrasound scan to assess fetal breathing movements , tone and amniotic fluid volume.
Hope my answer is helpful.
Do accept my answer in case there are no further queries.

Baby weight at 36 week is 2.37 kg
Is it ok ?

Is there any need to do elective c section at 37 weeks because of 2 loops around the fetal neck ?

Thirdly can the loops cause any harm to baby in the uterus before any labour or contractions ?
doctor
Answered by Dr. Dr. Rakhi Tayal (17 minutes later)
Brief Answer:
Details below.

Detailed Answer:
The baby weight is alright for gestational age.
Elective section at 37 weeks can be considered due to the tight loop of cord as a normal delivery is a less likely possibility. It is best to get a repeat ultrasound scan done after 37 weeks and in case the tight loop of cord is still present then consider going for an elective C- section.
The loops will not cause any problem before the pains or contractions start. Regular and normal baby movements will indicate the fetal well being.
Regards.

Thank you so much for the reply..:)
Actually I have a doubt about the loops as one of the doctors told me that it can cause compression of blood supply even if the labour does not start and I m confused and getting anxiety about this as this is v precious pregnancy through iui
Fetal movements are ok
Secondly I will be 37 weeks on 27 th and was thinking of elective c section on the same day..are the baby lungs fully matured by then ?
Can I take any test to make sure ?
doctor
Answered by Dr. Dr. Rakhi Tayal (5 hours later)
Brief Answer:
No such test.

Detailed Answer:
The loops of cord do not cause any compression of blood supply without any uterine contractions. So, do not worry.
The lungs of the baby are fully matured by 37 weeks. There is no test by which we can confirm this before the baby is born.

hi doctor,

thankyou so much for your reply

i went for scan today everything seems normal except amniotic fluid volume

it says there is a mild polyhydramnious and AFI IS 185 MM

IS IT DANGEROUS TO HAVE THIS VOLUME OF AMNIOTIC FLUID AND IS MEANT BY THIS FINDING?

I A ATTCHING MY REPORT ALSO
doctor
Answered by Dr. Dr. Rakhi Tayal (59 minutes later)
Brief Answer:
Not dangerous for baby.

Detailed Answer:
Mild polyhydramnios at 35-36 weeks is not worrisome. It can cause an earlier labor pains. This is not dangerous for the baby at all. It is good that now there is only one loose loop of cord around the neck. Now elective C section is not necessary. We can wait for a normal delivery.

I m sneezing a lot as I have caught cold I hope too much sneezing won't do any harm to placenta ?
doctor
Answered by Dr. Dr. Rakhi Tayal (6 hours later)
Brief Answer:
No harm to placenta.

Detailed Answer:
Baby and placenta are WELL PROTECTED inside womb and there will be NO harmful effects from mother's common cold sneezing and running nose at this stage.

Some basic home remedies can improve your condition and prevent recurrence:

a) Drink plenty of water or fruit juice to stay hydrated and keep throat moist.

b) Inhale steam or use saline nasal spray to ease nasal congestion and drippy nose.

c) Stay away from household dust, pollens, powders, droplets, smoke, allergen substances etc.

d) Take one teaspoon honey & tulsi juice in the morning or hot drink like milk at bed time is quite helpful.

e) Take hot shower bath, sleep with an extra pillow under head and vitamin-C supplements are very much essential.

Hello again,
i got a scan again today and amniotic fluid is still 188 mm
and baby weight is 2.6 kgs at 37 weeks
is it normal

also i am attaching the report please let me know if this is normal

my sugar is apsolutely normal
doctor
Answered by Dr. Dr. Rakhi Tayal (1 hour later)
Brief Answer:
Report is normal.

Detailed Answer:
I have seen the attached report. The growth in the fetus is normal. The mildly increased liquor volume is not likely to affect the baby. Since your sugar levels are normal there in no need to worry. Just wait for the spontaneous labor to start and continue monitoring the daily fetal movements.

Thanks a lot

But the head circumference is little less
Is it normal ?

Also in my last scan which happened in another clinic head circumference was 319 and one it is 300
How is it possible ?
doctor
Answered by Dr. Dr. Rakhi Tayal (5 minutes later)
Brief Answer:
Not worrisome.

Detailed Answer:
The decrease in head circumference can be due to variation in the measurement of the machine. Since the BPD is increasing normally there is no need to worry. AC is a better indicator of fetal growth in the third trimester.

hello again!!

my doctor is planning a c section on 4th oct i.e after 3 days and told me to take a steriod named BETAMETHASONE 12 MG for two days prior to c section

are there any side effects of this inj?

does it cause any effect on the baby heart rate?
is it safe for the baby?
doctor
Answered by Dr. Dr. Rakhi Tayal (6 hours later)
Brief Answer:
It is safe.

Detailed Answer:
This is a steroid injection and is given for the fetal lung maturity. It is safe for you and for the baby too. This helps in proper breathing for the baby after the birth of the baby. It will not affect baby's heart rate. Please do not worry and go ahead with treatment suggested by your doctor.
All the best.

When should this be take i mean if I have to deliver on 2 nd oct
Can I take today?
Or it should be given 48 hours prior?
doctor
Answered by Dr. Dr. Rakhi Tayal (13 minutes later)
Brief Answer:
It is given 48 hours prior.

Detailed Answer:
It is most effective if delivery occurs at least 24 hours after the first dose of the medicine has been given and less than 7 days after the last dose of the medicine. So it is best to take the injection 48 hours prior to the scheduled time of C- section.
Best regards.

thanks a lot
my c section is scheduled on 4th oct when i complete my 38 weeks
the baby has two loops again in my latest scan.
i shall again have a scan tomorrow...

if there are two loops the should i try for vaginal birth or should i go for c section only?
are there any side effects of c section for the next baby?

also are there breathing problems with baby delievered through c section?
doctor
Answered by Dr. Dr. Rakhi Tayal (3 hours later)
Brief Answer:
Details below.

Detailed Answer:
If there are two loops then it is best to opt for a C-section. It ensures a safe delivery. In case of next baby the chances of delivery by C- section are increased. Otherwise there are no side effects related to development or growth of the baby.
The breathing problems are seen if the baby is delivered before 37 completed weeks. Since you will deliver at 38 weeks there is no need to worry.
