Question: I have a very standard routine these days as I shelter in place. I work and exercise all day (tennis and power walking) then I have dinner and go to my couch or bed and marathon watch tv. So I%E2%80%99ve noticed after a few hours (3-6) sitting down or reclining in bed my toes on both feet tingle and neither big toe. Just the other four always in both feet. Never in my upper body but occasionaly up into my calves. Is this normal. I do get up to go to the bathroom but I%E2%80%99m usually in one position for many hours. Just curious. it doesn%E2%80%99t hurt like sometimes when I sit on the toilet longer than normal and my feet go to sleep and then intense pins and needles. Just curious.

I have gone through your query and here is my opinion.

It might be an early sign of a neurological or vascular problem.

Tingling, numbness,burning and pain in feet can be a sign of pressure or chemical injury to nerves (Neuropathy).
The damage may be related to a single nerve or group of nerves.

Diabetes is the most common medical condition associated with nerve damage.
Other causes include heavy alcohol consumption,nutritional deficiencies,thyroid disorders and some medications etc.

A neurological examination and additional testing like NCCV ( Nerve conduction velocity) EMG (Electromyography),blood work (Serum Vitamin D and B12 levels,Thyroid Profile,Blood sugar,HbA1c and complete blood counts) and sometimes spinal MRI are indicated in such patients.

So please explain why it only happens during prolonged sitting and reclining or in bed. Seems to me, PHD in Clinical Psych, that if it were Peripheral Artery Disease or some type of neurological disorder, it would be more pervasive. My gut feeling is it’s relayed to protracted time in one position with my head and neck in an unnatural position. If I can play competitive tennis for 90 minutes in 90 degree heat I doubt it’s my heart. Btw, I see a cardiologist annually and have a complete work up inc Holter and echo every 3 years. This is all due to a single afib event back in 2013. I take 2x80 mgs Sotalol a day. No blood thinner. My only XXXXXXX number is I’m a man. Thx

As there is no Intermittent claudication(pain or cramping in the arms or legs that occurs with exercise and goes away with rest) or rest pain and there are no signs and symptoms of vascular occlusion (change in color,cold feet,absence of pulse,sores etc) so it is unlikely to be a Peripheral Artery Disease.

As these symptoms are not a presentation of any cardiac ailment and you are on continuous cardiac follow up,a cardiac pathology has already been ruled out.

Unnatural positions and prolonged sitting and reclining,might cause excess of external pressure on nerve twigs,thus precipitating the symptoms.

You need a complete neurological examination and additional tests like NCCV,EMG,Serum Vitamin D and B12 levels,Thyroid Profile,Blood sugar,HbA1c and a spinal MRI.
Kindly visit a neurologist.
