Question: I had a urine test done, which cane back that I have a uti from a recent issue with diverticulitis. They will send it off for further test. One of the test they mentioned was for bilirubin. I was diagnosed with XXXXXXX in November 2018, through and ultrasound and alt result of 42, ast result of 31, and bilirubin result of 0.3. Can the fatty liver cause a positive bilirubin result on a urinalysis test?

Brief Answer: XXXXXXX rarely causes high bilirubin and related changes in urinalysis, other causes need to be ruled out first

Detailed Answer:
Hello and welcome to 'Ask a Doctor' service,

I carefully read your query and understand your concern. Sometimes when patients get high bilirubin levels in the blood, this may be reflected in the urinalysis.

Generally, high bilirubin is uncommon with XXXXXXX (non-alcoholic steatohepatitis), thus I would not expect the findings in the urinalysis to be due to this.

The doctors will need to do further investigations to rule out other causes of high bilirubin first before XXXXXXX is considered as a cause of the urinary findings (as it is rarely the cause).

I hope this answers your query.
I'm at your disposition in case more medical assistance is needed.

Thanks for the information. Very helpful. I was very nervous about the urinalysis test of the bilirubin.
I’ve been working hard to reduce my fatty liver, by changing my diet and excercising. I’ve lost 50lbs since November. Once again, thanks.

Brief Answer:
Blood tests are needed to confirm this finding and possibly the cause

Detailed Answer:
Hello and welcome back,

Thank you for uploading the results. Bilirubin levels are mildly elevated (+1).
Urinalysis may show increased bilirubin but to make sure of the liver function blood tests need to be done.

I would advise you to have the following blood tests:
- ALT, AST, bilirubin in the blood (the conjugated and the unconjugated form), albumin levels and CBC

If liver tests come out abnormal, the specific findings will guide to diagnosis or to possible further needed investigations to determine the exact cause of the increased liver enzymes.

To conclude:
- the higher than normal levels of bilirubin in urine indicate that further more specific tests should be done to confirm that there is a problem and if so, which is it.
- according to the blood tests, the doctor will be able to determine what is going on

I hope this answers your query. Let me know if I can assist you further.

Kind regards,
Dr. Antoneta Zotaj, General and Family Physician
