How To Determine Ovulation Dates While On Clomid And Letrozole?

Question: hi
this is my second cycle of medicine first month i took clomid and this month i took letrozole and finally i had two follicle one was 25mm and other was 19mm and et was 8.5mm and they just got ruptured yesterday and i also got hucog 5000 injection 3 days back and me and my husband also had sex from last three - four days and my doc has recommended to have sex 2 more days and she has prescribed me duphaston for 14 days twice in a day is that ok to have and also i wanna know
we had sex 3-4 days prior to rupture and on the rupture day and doc has recommended 2 more days that won't reduce his sperm count and what are the chances of pregnancy

Detailed Answer:
Dear XXXX,

In fact, the egg has a life of 24 hours and the sperms are active for 3 days. So the intercourse(s) on the day of egg release and within 24 hours of the egg release are critical for the sake of conception. Any intercourse more than 24 hours after the egg release will not be useful as far as the conception is concerned. Any intercourse 3 days before the egg release can be useful for conception.

Duphaston is a progestational medicine. Progesterone is required for stabilizing the pregnancy throughout; hence this advice. It will help. However, it should be continued only for 14 days and then the pregnancy diagnosis should be confirmed. If the conception has not occurred and still you continue Duphaston, you will not get menses. The best option for you is to go for Beta hCG test 10 days after last unprotected sex after the egg release. Values more than 25 mIU/ml are diagnostic of pregnancy while in non-pregnant state, the values are less than 5 mIU/ml. If you are not pregnant, you may discontinue Duphaston to have withdrawal bleeding within 4-7 days then after.

1.we did have sex continuously for 3 days prior to my follicle rupture and yesterday also when i got to know that my follicle has ruptured and my doc has recommended 2 more days of sex my question was that won't reduce his sperm count
2.how much chances are there for pregnancy and is there any other way i can get to know right now if i m pregnant or not ?
3. when my ultrasound doc told me my egg has ruptured i came home and did opk but it came negative i checked it again both the times it came negative

1. You had daily sex for three days prior to ovulation; moreover, you had sex on the day of ovulation (after ovulation). So yes, you do stand a good chance of pregnancy. You need not worry about reduction in sperm count. When the semen quality is good, some reduction of sperms does not matter.

2. The earliest practical method for detection of pregnancy is by Beta hCG test 10 days after the last unprotected intercourse. Please wait for this period. Anxiety is not going to be anyway beneficial.

3. Ultrasound has provided direct visual evidence of ovulation. It is more reliable than OPK. OPK gives inference of ovulation based on the hormonal changes. Rely more on ultrasonography.

It is likely that your LH (hCG) is on lower side, hence the negative tests. hCG is needed for maintenance of pregnancy till the placenta takes over. However, you have been given Inj. hCG 5000 IU which will take care; moreover you are prescribed Duphaston also.

also since the time i had letrozole i have major major hairfall my hair has become half of what it was i can't express in words how bad it is what can i do as i don't know if i m pregnant or not and i can't even put anything and by the time i will get to know i believe i won't be left with any hair its that bad what should i do i m having my supplements folic acid and fish oil
please help

Loss of scalp hair is not a reported side effect of Letrazol. However, in PCOS, since male hormonal levels are increased, hair distribution may be like that of a male pattern leading to loss of scalp hair and appearance of coarse hair on unwanted places like face, chest, abdomen, legs, etc. Have high protein diet with low carbohydrates and low fats. Consume goof fresh fruits and green leafy vegetables.

Myo inositol is a medicines which helps controlling the rising levels of both, insulin and male hormone. You may suggest your Gynaecologist. This medicine also helps Clomiphene citrate in induction of ovulation.
