Question: Hi, Had unprotected dry sex near to period date - this was about 24 hours ago. There was no entering and chances of serum entering vagina very very low. Still wanted to take ipill just to be safe. I have PCOS and am not on any medication for it. Am worried about side effects of i-pill as I normally also have irregular periods, heavy bleeding for 5 days, high weakness and cramps due to PCOS. Will ipill make it even worse? Will ipill aggravate PCOS? I dont want to be preganent - considering my PCOS history should I take i-pill in this scenario? Have never taken any such pill before and am 21 years of age.

Brief Answer: TAKE I-PILL TO AVOID UNWANTED PREGNANCY. Detailed Answer: Hello, Thanks for writing to us. Followings are my comments: 1) As you have PCOS (Poly Cystic Ovarian Syndrome) with irregular period, then it is quite DIFFICULT to calculate exact SAFE period and RISK of pregnancy is there from unprotected sex with PRE-EJACULATORY fluid or Seminal fluid. In this scenario, I suggest you to take I-Pill at the earliest to avoid unwanted pregnancy. It will give more than 95% protection within first 24 hrs and around 85% within 24 - 48 hrs. 2) You will get a withdrawal bleeding within 5 to 7 days of taking the pill. It is a quite common and natural phenomena for most women. However, your next period may be delayed for 1 to 2 weeks due to hormonal imbalance and mental stress. It is wise to undergo one home pregnancy test 4 weeks after last sexual intercourse if you don't get your period in this time. 3) Lastly, I suggest to consult with Gynecologist for other investigations and treatment of PCOS. Combined hormonal pill and supportive medication are usually given to treat the condition. It will be helpful for you as combined pill will give REGULAR monthly period and HIGHEST protection (around 99%) against unwanted pregnancy. In addition, you must take LOW carbohydrate diet with more vegetables, do regular physical exercise to reduce or control body weight, avoid mental stress, drink plenty of water and take sound sleep. Thank you very much Doc, I intend to take the ipill as suggested. I especially liked your guidance on diet and exercise and intend to follow that :) I have one question though I am having stomach pain right now and also body pain like I normally have when I have my periods. As per my cycle I should have had my period on 14th March (Just FYI the incident was on 16th - I think the period is delayed like usual because of my PCOD). The stomach pain is so much I think I might get a period in the next 6 to 7 hrs (i.e. tomorrow morning). Would it be wise to wait till the morning and see if I naturally get a period. If yes, then I dont need to take the ipill - right? Should I do this? (Its already been 33 hrs from the incident - if I wait till morning - it will be like 40 hrs). Or will waiting for for this 7 extra hours drastically reduce the success rate of ipill in avoiding pregnancy??? Avoiding pregnancy is my top priority but because I normally have super bad periods I am also scared of the side effects of ipill! Could you please elaborate a bit on the side effects of ipill on a PCOD patients? Please guide me as soon as possible - I am staying awake for your reply.

Brief Answer: YOU CAN WAIT TILL NEXT MORNING TO GET PERIOD. Detailed Answer: Hello, Thanks for follow up query. 1) If you have abdominal cramp right now and might get period soon, then you will be 100% SAFE from being pregnant and there is NO need to take I-PILL. However, if you don't get your period in the next morning, then it is better to take the I-PILL in benefit of doubt within 48 hrs for maximum protection. 2) Effectiveness of the I-Pill will remain same if you take that within 24 to 48 hrs (i.e. within the next morning) and not beyond that.
