Question: Hi I delivered a baby 4 and half month ago his posterior fontanelle closed only 90 percent when the remaining will get closed

I can understand your concern.
Generally posterior fontanelle closes by the time of birth, but in some cases small opening may be there which you can expect to close at around 4 to 6 month of age.
If the weight gain and other developmental milestones are normal I would suggest you to wait for some days.
You can see a doctor to rule out hypothyroidism (do check the blood fot T3, T4, Tsh).

Loose stools which are passed frequently are common in infants.
It is not the frequency which matters but it is the consistency (watery and foul smelling) of stool which is more important, to say that it is diarrhea.
You can do stool examination to confirm whether it is due to some infection.
If the child is playfull, feeding well , no fever, passing adequate urine(6 to7 wet diapers in a day), then nothing to worry.
You can give him probiotics like econorm XXXXXXX once daily in mothers milk for 4 to 5 days.
Also syp. Zn-20 (zinc) may be given 2.5 ml once a day.
If not relived see a doctor.

Thanks doctor my child is playful feeding well n passing urine at frequent intervals but stools are little watery but not foul smelling and it is very less in quantity only one small tea spoon level he is passing stool that too one hour once that too not in night should I need to take paediatrician

As you said the child is playful, you can wait for 2 to 3 days.

Till then you can start the treatment which I have suggested in the previous reply (econorm and zinc).

If not relieved in 2 to 3 days then do stool routine and microscopy before going to doctor. It will save your time and doctor can directly prescribe you the required medicine after watching the report.

Hi I checked with my doctor he said nothing to worry n given enterogermina oral tube and he said to take one full tube morning and evening is it a right dose n regarding posterior fontanelle he said it'll completely closed once he turns one and also I have one more query skin which is in and around posterior fontanelle is very soft like the skin in our hand and leg is it very normal

Enterogrrmina and econorm are probiotics you can give any of them.
Dose advised by your doctor is right.
Wait for closure of fontanale till 6 to 8 months otherwise do thyriod test.
Skin around fontanale may be soft in infants, nothing to worry.
