Question: 65 yr old female, large CA125 negative right ovarian cyst. My OBGYN is having a hard time deciding
course of action to take....laparoscopy vs hysterectomy. This was found on routine yearly exam then
ultrasound. Thoughts?

A large right ovarian simple cyst with normal CA 125 is less likely to be related to a malignancy. Since the uterus is normal, hysterectomy is not needed. Laparoscopic removal of the cyst is the better option followed by a biopsy which can rule out a malignancy further. Hysterectomy is a major surgery and should be done only if needed.
