Brief Answer:
Needs Cardiac Evaluation

Detailed Answer:
Hello Sir

After going through your medical details I understand your concern for your health and I would like to tell you that heart rate and blood pressure are ever changing phenomena depending on your activity level, emotional stress and physiological changes but if you are experiencing chest pain which increases on exertion that can be indicative of underlying heart disease which requires proper evaluation.

Being a diabetic you are more prone to have cardiac health issues than a non-diabetic patient. So you need to get a basic cardiac evaluation which includes

1. ECG

2.Echocardiography

3. Treadmill test

4. Lipid Profile

Kindly get these tests done and revert back with reports so that I can guide you properly.

Kind Regards
Dr Bhanu Partap
