Question: Hi, this question is for my wife who is 39 years old and ~11 weeks pregnant with our second child (our first child was born low birth weight and 3 weeks early and originally had feeding difficulty but is mostly ok now).

We conceived the 2nd child through IVF. The 10 weeks ultrasound looked normal but her blood work showed some issues, which I have attached and wanted to get your opinion. Thank you.

Welcome to Ask a Doctor. I am Dr Ramadevi Wani. I will be answering your concerns today.
I have gone through your wife's reports. The red cell count is normal. The white blood cell count of 13.6X10(3) is considered normal during pregnancy.
The slight increase in thyroxine level is normal in the first trimester due to increase in pregnancy hormones.

Why was beta2 glycoprotein IgM test ordered? It is part of the test panel for antiphospholipid syndrome. It is usually ordered along with lupus anticoagulant and anticardiolipin antibodies. Its' presence is usually associated with miscarraige, excess clotting in the body etc.
Protein S activity is also marginally low. This too increases the chance formation of blood clots.

Did your wife have any miscarriages before. Discuss with your doctor about these reports. She/he may start her on low molecular weight heparin. I presume your wife is already taking low dose Aspirin.

Hi, no she is not taking it yet but her doctor said to start beginning of second trimester. How will it help? She had no miscarriages before but son was low birth weight.

Were your son delivered at 9 months? What was his birth weight?

Aspirin improves Placenta formation and blood circulation within it. This helps the baby to get enough nutrition.

You may discuss it with your doctor and start the Aspirin tablet now.

As I said earlier, low protein S level and presence of Beta 2 glycoprotein make the blood more prone for clotting. Aspirin counters this effect and improves placental circulation.
