Question: Hi, I have digestions problems that seems to collide with other symptoms and wonder if there might be a bigger picture. I was diagnosed with IBS at 14-15 years old, and with GERD and esophagitis at 20 years old. I am now 25 and have been on strong PPI's for 3 years and the Japanese Takecab 20mg for 2 years. Despite this I have worsening symptoms of heartburn, the esophagitis is still there, (from endoscopy) I also have mild gastritis. I have also been troubled with irregular heart rythm, episodes of slow heartbeat, varying blood pressure and anxiety. Recently I have been experiencing nausea, headaches, trouble concentrating and irritability. I experience strong heartburn and headaches between meals that gets better from eating.
My physician is trying to blame it all on stress and anxiety. But I have personally felt these aspects of my life getting better. I am wondering if i might have SIBO, or problems with my ANS or something else which might account for my symptoms. I am a female, I have taken birth control pills for 1 year, I have had chronic iron deficiency these last 2 years, I have normal weight, and have lost weight not on purpose 2 years ago ( about 5 kg) I haven't felt completely healthy for 10 years.

I am sorry to hear about your troubles. I suspect that anxiety and/or stress could exacerbate your symptoms. The irregular heartbeat and blood pressure should be investigated further, with blood tests and ECG. In regard of the heartburn, you could try something practical, e.g. sleeping with elevated headrest, avoid late meals, no smoking or eating spicy or smoked foods.

I sleep elevated, I avoid all triggers, my heartburn is not normal. I get heartburn sitting or standing up, and before meals more than after. I have done the ph test where they put a tube down your nose for 24 hours, I had no signs of heartburn. But at the same time I have 3 endoscopy tests over 5 years all showing inflammation of the esophagus. I was on both Zantac and Nexium at the same time for a period of over 2 years without full relief, and the pain just slowly gets worse. I am taking a PPI in Japan now that is supposed to be very good, but it's the same thing. The last week I have been having constant heartburn, slightly better in the morning, but awful at night.
I only have short panic attacks easily managed by talking to someone, this time it's a week of worsening symptoms.
I used to have very stressed day with 3 jobs and school, but from this year everything calmed down, I'm not stressed about anything but my failing health.
No one wants it to be just stress more than me, but it just feels different. I have IBS, I'm used to stress related health problems.

Thank you for elaborating on your symptoms and history. Have you had biopsies taken from your esophagus? If so, what did they show?

Also, an uncommon presentation of asthma should be considered, especially if you have respiratory symptoms (?).

They did a test for H. Pylori the last two times I went for endoscopy, they were negative. I don't know if they took biopsy or blood test though.

Regarding breathing problems, I sometimes get a very strong cough which can last weeks even with medication, I was told it was allergy related. My mother has mild asthma.
Sometimes at night I feel like I can't breathe properly, but I always felt it was just how I slept, it always gets better when I change position.
Apart from that my lungs always sound very nice I'm told.

It is probably nothing, but I find it relevant for you to get tested for asthma. If you ever another gastroscopy, biopsies of your inflamed esophagus could help differentiate different kinds of inflammation with implications for treatment.
