Question: Hi, I have been to Urgent Care twice in the last 10 days. I am suffering with anal/vaginal itching. They have been treating me for a yeast infection. They have prescribed Ketoconazole cream but it is not helping. I have scratched until I am raw. Is there a home remedy or should I head to a local ER?

Brief Answer:
Better to go to the urgent care and get the issue looked into afresh.

Detailed Answer:
Hello and welcome to Ask a Doctor.

I have read through your query and understand your concern.

I am so sorry to know how much you are suffering due to this.

From your description it appears like there is a mixed fungal and bacterial infection.

If I was your treating doctor I would prescribe an antifungal cream (like ketoconazole) along with an antibacterial cream like mupirocin.

The itching will subside by taking an antihistamine such as benadryl twice a day.

Avoid wearing undergarments that can stay wet.

Keep the area dry.

After passing stools, sit in a tub of slightly hot water for about 5 minutes, then dry with towel and apply the antibacterial and antifungal creams.

If the infection is severe, I would prescribe oral antibiotics and antifungals.

I think it is wise to go to the ER if the discomfort is so severe as to make you scratch so much.

I hope this information will be help to you. Feel free to write back to me if you have any additional concerns.

Best wishes.
