Question: Hi Doctor,

Recently when i got an MRI of my abdomen for diagnosis of Cushing, it came out that I had 17 cm of fatty liver. I am 5.8 high and i weigh 74 kg of age 24. I always been slim and skinny all my childhood, 3 years back I was diagnosed of Gynecomastia and since i had developed fat over my abdomen area and chest but other parts of my body still remain skinny though. I don't drink alcohol nor smoke nor been on any drugs all my life. I look under grown and small compared to people of my age. Other symptoms which i had are day dreaming, not able to concentrate for long, getting tired easily, slight depression, not able to exercise, feeling week.



Lipid Panel:

Total Cholesterol - 209

HDL - 48

Tri-glycerides - 171

TC/HDL ratio - 4.4
LDL - 127

Glucose(non fasting) - 92

BMI - 24.6

Body fat% - 24.2





My question for you are:

1) Is fatty liver of 17 cm usual and normal or bad or worst for my age

2) What should I do now , are there any surgeries i need to go through or medications i should be taking

3) Can this fatty liver also a symptom of long untreated Cushing disease?

4) should I have go for treatment of my fatty liver first or cushing?

Have seen your details and I appreciate your concerns,
The condition you are suffering from is called skinny fatb in which body deposits fat around tummy area while rest of the part is skinny.
Answers are-
1)It's ok at your age.
2)I would suggest you to consult a psychiatrist for weekness fatigue depression like symptoms however gynecomastia can be corrected by surgery only. A thyroid function test can be done to rule out hypothyroidism.
3)Yes it can be a result of untreated Cushing disease. Proper treatment under endocrinologist should be taken to manage Cushing disease.
4)Treat Cushing disease fatty liver would be cured automatically.

Doctor,

Except the fat I gained on my body, the other symptoms which I mentioned has been struck from childhood. I didn't notice those as an issue earlier and now as they are getting worse I can see how its effecting me in daily activities. It's not just the aesthetic issue for me but I feel that even now I look under grown and I don't see any growth in my muscles even after certain workouts in gym.

I took some blood tests which are negative to thyroid, please check the report attached now. I also took an MRI of head which showed no signs of pituitary tumor, my adrenal glands also indicate nothing of that sort, so did my 24 hour cortisol urine tests. But I did have some high cortisol in midnight saliva tests. I will be taking dexamethasone suppression test next week. I didn't have a XXXXXXX hump or fat on my face but I do have some pain below my abdominal area.

Can fatty liver be symptom any other disease/tumor in my case?
Seeing my reports do you think this can be cushing?

Hi there sorry for a delayed response. As per the query regarding possibilities of Cushing's it's dealt by an Endocrinologist therefore suggestion is to post the query on respective speciality.
Now let's move on to fatty liver.The problem is mainly due to your weight related issues.Drop your weight take a healthy diet and it would be ok.
Fatty liver though is a normal problem and I don't feel has anything to do with your problem.
Further you should consult an endocrinologist for the same.
