Hello I Am A Fit Individual With Almost No Known

Question: Hello
I am a fit individual with almost no known diseases. Normal BP Blood sugar. Dont smoke. Very light drinking on social occasions. But i do have WHITECOAT HYPERTENSION (PASSIVE) in hospital and clinical settings . Externally i am ok , but in the background my Heart rate rises to 100-110 bpm in Medical / Hospital settings which induces some increases in BP also .
What i have seen is the following
BP in hospital settings in many visits is 145/90 , while at home in many years of monitoring my BP is normally 110/75 in average.
One of my doctor cousins told me that i should have IVABRADINE 5mg if i am visiting any Medical Centre or Hospital to get my heart rate down in Medical Settings. Might be needed to get some medical fitness certificates and all.

Can you advice if IVABRADINE is safe to take for me in a 5mg per day dose , before visiting any medical centre or Hospital.

Otherwise i am very fit , run- 3/5 km , jog , play sports etc regularly.

Hello!

Welcome to Ask a Doctor service!

I passed carefully through your question and would explain that it is generally safe to take Ivabradine 5mg before going to the Hospital or consulting with your doctor as it may reduce the heart rate without affecting the heart function.

Anyway, I think that it would be better even not taking this drug.

Another option to help reduce anxiety before consulting with your doctor would be taking one tablet of Alprazolam 0.25 mg half an hour before consulting with your doctor. This drug can help reduce anxiety and it has no effects on the heart rhythm and function.

Hope to have been helpful to you!

I remain at your disposal in case of further questions whenever you need!

Hello I think Alprazolam is not available in the country we live in (UAE). Kindly suggest alternatives in this case. Ivabradine is available here.

I would explain as follows:

Detailed Answer:
Hello again!

I checked the list of drugs used in your country. You can find alprazolam under the name of Xanax 0.25. This is really a low dose of alprazolam and its effects usually go away after 6 hours.

I would recommend using one tablet of Xanax 0.25 mg half an hours before consulting with your doctor. I would rather prefer Xanax to Ivabradine.

Anyway, the choice is up to you! Both these drugs are safe to use.

Hope you will find this information helpful!

If you have any other questions, please don't hesitate to ask me again!

Wishing all the best,

Dear Doc
I will prefer to go with Ivabradine as its OTC.
Kindly advice me on following
1. For how many days i should take this medicine. My appointment is on 19th XXXXXXX and i need my HR and BP to be not fluctuating in hospital settings on that day.
2. Should i monitor anything post having this , HR or BP ?
3. The dosage should be 5mg once on the day of visit in the morning ?
4. Any Side effects ? I should watch out for

Thank you

Brief Answer:
I would explain as follows:

Detailed Answer:
Hello again!

I would recommend taking this drug once a day (5mg), in the morning, starting two days before consulting with your doctor.

The main side effects would be a low heart rate and low blood pressure, dizziness and fatigue. If you experience any of these adverse effects, it would be OK to reduce the dose to half a tablet.

It would be recommendable to monitor your blood pressure values and heart rate those days that you will take this drug.

Hope to have been helpful!
