Question: Hello Dr XXXXXXX . I had gastritis about 6 months back.As you suggested, I started with 2 nexium 40 mg each . I have slowly scaled down to one 20 mg nexium in the morning and one 75 mg zantac in the evening.One or two days a week, I take 80 mg nexium in the evening before having 3 or 4 light beers. Then I have 2 tums in the end.It is working. My questions are:

1) Because of adverse Zantac news, I want to switch to pepcid in the evening. Is that ok or you suggest something else. what dosage of pepcid will equal 75 mg zantac?

2)Instead of 80 mg nexium in he evening when I am drinking, should I try only 40 mg or 60 mg nexium?

3)Is it better to have tums before drinking instead of afterward?

4)I still have to take these medicines. Should I take 20 mg nexium in morning and also 20 mg in evening instead of nexium plus 75 mg zantac? will total 40 mg nexium be better and heal me faster.

5) Are 2 to 3 pegs of single malt whiskey better for gastritis than 3 to 4 light beers?

1)Absolutely yes Pepcid 20 is ok.
It belongs to the group of Ranitidine only.

Pepcid 20 mg is ok.
2)You can give it a try.
The minimum possible dose you are stable on is best for you.
3)No change.
4)Depends totally on which drug suits your body more.
Gastritis can't be cured it can just be controlled. So there is no rule..Only thing is to keep patient on minimum maintainance doses.
5)Read the alcohol content of both beer and whiskey which ever has more alcoholic contents in it; you need to avoid it.

5) Should I eat more protein to heal gastritis. Right now I eat 60 gms a day. My height is 5'10". Weight 173 lbs.

3)No as explained earlier we can't heal or cure gastritis our aim is just to control symptoms.Therefore if low dose is helping you there is no need to increase dose in necessarily.
4)Why do you want to change medicine just for 10 days? Reason?
5)Protein is going to irritate your gut more. Its difficult to digest high amount of proteins.
Keep yourself on a 50:30:20 carbohydrate protein and fat diet.
Take protein equal to your weight in kilograms per day.

1) You mentioned I can go to Rabeprazole 20 mg which will need to be taken only once a day. Rabeprazole is aciphex which is used for ulcers. It is very strong and should not be taken long term. Is my understanding right?

2)17 years back, I had a strong burning sensation in my stomach. My doctor gave me aciphex for 10 days and the stomach was fixed. So I thought may be aciphex can heal my stomach better than nexium.

3)Next I had gastritis in 2012 and it did get fixed completely and totally. I started with aciphex( Not sure) but then took dexilant for months. Then I switched to 2 zantacs 150 mg and got completely fixed again. I do not have problem of excess acid production in the stomach. The stomach lining got damaged due to excess magnesium intake for chronic constipation. Any thoughts will be appreciated.

Hi,
Rabeprazole is aciphex- Sorry trade name varies from country to country thousands of companies sells the same salt with different names.
Rabeprazole is no different from Esomeprazole(Nexium) both of them are proton pump inhibitor.Only significant difference is Rabeprazole works for 24 hour while Esomeprazole works for 12 hours(so is given twice a day).

All proton pump inhibitors are strong.
There is no such thing like rabeprazole is more stronger then esomeprazole or pantoprazole.
They all belong to same class of medicines called proton pump inhibitor.

Going by book long term(10years plus on high doses like 80 mg twice for nexium) use may cause mild issues with kidney but fact is that in that case we can't give you any medicine.
Any medicine given to you is a chemical and is going to have some side effect.
In real world this is the most commonly prescribed group of medicine.

2)In that case you could switch over to Rabeprazole.
As i said both belong to same group of medicine.
Sir 17 years back you were in early 50s and now you are in 70's ......A young body would respond to any medicine far better then old body.
As said earlier Gastritis could be controlled and not cured.If gastritis is due to a bacteria named H.pylori then only it can be cured.

None of the proton pump inhibitor heals any thing it just inhibits the secretion of excessive as well as baseline HCL in stomach.

3)Magnesium salts(especially hydroxide) are itself an antacid.
They are prescribed in combination with aluminium.
Gastritis as a result of Magnesium used in laxatives is some thing very rare/I have never seen such case.

Dexilant again is Dexlansoprazole and why your doctor switched you from rabeprazole to it is his/her decision. I am sorry but no doctor could make a comment on that and if some one gives you a justification(as patients are lay person) then he/she is just not talking about facts to you.
As said earlier it belongs to same class of medication.

Yes Zantac is Ranitidine and is H2blocker-They are inferior to PPI's as they blocks only excessive acid secretion and have no effect on baseline acid secretion.
Its safer then PPI'S in long term use but again the effect is not as good as PPI's.

Follow ups are welcome.
Two Doctor would never reach the same conclusion in any patient and every doctor has his/her favorite sets of medicines based on his/her experience.
