Question: Hello Doctor,
pls find attached blood and abdomen scan reports for elder twin, same was advised by local doctor. kindly review and advice on the reports.

Thank you for consulting me on Ask A Doctor.

I have gone through the medical history provided by you and the attached reports.

The abdominal ultrasound scan is normal.

The blood report is showing anemia (low haemoglobin level) and higher blood cell turn over (red blood cells seem to be getting destroyed faster and new cells are coming into circulation faster). This could indicate low vitamin B 12, low vitamin C or low folic acid levels which are all required for proper production and maturation of RBCs.

This will require further testing to see if the RBCs are getting destroyed.

You can ascertain that by doing bilirubin levels. And peripheral blood smear study.

Apart from that, the white blood cell number is increased, which indicates ongoing infection.

Could be due to Malaria, or maybe inborn error of metabolism.

I am presuming here as you have not mentioned why these tests were requested by the local doctor.

Please clarify that so that I can guide you further.

Hello Doctor, thanks for the response. There is speech delay in my son, so we took him to few doctors / specialist, some are saying my son has mild autism features, but can't say right now. while some are saying they don't see any features. hence we took him to another doctor for same.
we told the following to the doctor 1) attention deficient, 2) does not respond to his name quickly 3) gets very tired and looks week 4) delay in his miletones 6) speech delays 5) bloated stomach, hence doctor had recommended for these test.

we were already giving him ultra vitamin d3 & Tonoferon (colloidal, iron, folic acid & vitamin b12) syrup.

Doctor asked to stop this syrup and start giving Neuromin -M (piraceam & mecabalamin) syrup for 6 months.

kindly advice if we need to do further blood test as suggested by you.

I have noted the history carefully.

Developmental delay is a issue that needs careful evaluation by a paediatric neurologist.

The most important risk factor in your son's case is, he is a twin, and twins are usually born before their due date which makes them particularly vulnerable to trauma during delivery or perinatal hypoxic ischemic encephalopathy.

To complicate things, twins also have higher chances of acquiring genetic mutations and most genes have a role to play in the development of brain, so that also needs to be ruled out.

Please do get your child evaluated by a Paediatric neurologist and proceed with imaging studies like MRI of the brain to rule out structural anomalies of the brain.

Genetic factors can be considered at the end of imaging studies.

Inborn errors of metabolism can also present with similar features, so they also need to be ruled out.

Thanks for the suggestion doctor. Both are non identical twins. Post delivery elder twin was admitted in ICU for heavy breathing and high heart rate. He was under ICU observation for 1 day.

As suggested will have further test done.
