Question: Hello Doc,
My name is Zain. I am male, 30 years age.
Currently i am extremely obese. My height is 5,8 and i weigh
135kgs.
I have been suffering from anxiety disorder past 12-13 years.
It all started with a panic attack while i was driving to the
college.
Have been taking many SSRI, s and benzos past these years.
Literally i have tried almost every anti-depressant.
Currently i am taking zoloft 150mg at night, inderal 40mg
three times a day. And xanax 1mg (as per my anxiety. Its like
almost 3mg per day)
Now coming to the main point.
The problem i am facing these days is shortness of breath
after just walking 10-12 steps. I can not even stand for 20
secs and my lower back starts to hurt alot. I literally sweat
up for just after a few steps and end up being sitting on the
floor somewhere due to extreme shortness of breath.
Thad all my blood tests done including CBC, LFT, S. RFT,S.
Uric acid. Thyroid, vitamin B,s and a few more. All the tests
were fine.
Kindly guide me for my problem. I know i am too obese but i
do see people much fat than me running miles everyday.

I read your query and here is my advice.

I think you should even check tour heart function and cholesterol, triglycerides and LDL, HDL.
I think you may need to do an ECG to check for possible pathologies of heart causing the shortness of breath and chest pain. Also you may need to do a spirometry to check lungs function.
If every examination comes out to be normal then the shortness of breath may be caused only by obesity and anxiety.
A heart function evaluation is needed before you start intense physical activity needed to loose weight.

The test you have mentioned are all done and are perfectly normal.
I did have a lung test which is done prior to sleep apnea test. ( i forgot the name ). It was like a device in which you have to blow in with full power. The result was 90 percent right.
I have gained this weight all past 2 years. When i was 90kg i still had the problem of extreme sweating and shortness of breath while walking.

i just saw the video of spirometry test on youtube and yes it is the test i took before the sleep apnea test. as far as i remember the doctor said that the graph was 90-95 percent ok and was fine. do i need to take it again?

thank you for your reply. i just forgot to tell you one thing. i used to take anabolic steroids few years back to loose weight. i came down to 70kgs from 120 kgs with diet and steroids. the steroids i took were testosterone and trenbolone. i think we should also do the complete blood tests related to these steroids. there may be a possibility that i have messed with the harmonal system.
kindly advice me which blood tests to get done.
