Question: Good afternoon doctor - my medication as of now is:
Amitryn 135, quitipin sr 100+50, mirtaz 15
I have been on amitryn on 135 for the last 15 days, i am feeling much better in terms of anxiety. I was on amitryn 300 some 5-6 months, then started gradually tapering 5 mg every 4-5 days and brought it to 135 and with very reduction i felt better. To make it 145 t0 135(10mg reduction instead of 5mg), i did that in a span of 4-5 days i could feel the withdrawal symptoms, though my anxiety was much better but i could feel the withdrawal till day before. Yesterday and today i am feeling better. With every 5 mg reduction earlier, i would feel sleep disturbance for 2-3 days and it would feel better after that. 10mg reduction from 145 to 135, disturbed my sleep for 10-12 days coupled with body aches, irritation and appetite disturbance, feeling much relieved since yesterday. I feel, a little anxiety that is left can become better with a further 5 mg reduction (from 135 to 130) but little worried about withdrawals. Was thinking if i can cut 10 mg amitryn into 1/4 and make it (130+1/4 of 10mg), can i cut 10mg pill into 1/4th, is it ok to cut amitryn tablets as they dont have scoring. Or else, i was thinking of taking 130 one day and 135 the next day and do that for a week and then make it 130 after a week. What do you think?
I also feel, reducing 5 mg when i was on high doses did not affect me badly. Its only after i have started reducing 5 mg on lower doses (150mg and below), it has started giving me withdrawals. i dont want to tweak any other medicine as reduction in amitryn is helping me. Once, it feel totally stable with amitryn, will need your advise to tweak other medicines. Thank you. Regards, Rajesh

Hello again

The tablet can be divided even if it’s not scored but cutting 10 mg to one forth is not a good idea. You can reduce the dose by 5 mg. This would not affect much. Stop worrying for this.

But if you are not convinced then better to reduce to 130 and 135 on alternate day for a week and then take 130 mg daily.
