Question: I like to ask for an Psychiatrist for more information to quit one of medication-doxepin which I have been taking since 86.
I like also to know why I feel so tired and sleepy after eating?

Hello thanks for asking from HCM

You have been taking Doxepin since 1986. Doxepin is a tricyclic antidepressant drug and to quit drug one must taper the doses gradually over 8-12 weeks.

I would advise you to reduce the doses by about 20-25% every 2 weeks and then stop completely at about 8-10 weeks. If you would reduce the doses gradually serious withdrawal symptoms would not be observed and you will be able to quit the drug easily. If insomnia occurs due to reducing the dose for some time low dose benzodiazepines can be used on SOS basis to minimise withdrawal. Visit your psychiatrist for prescription.

The drug is known to induce sleep and this could be the reason for sleepiness and tiredness after eating. Sleepiness and tiredness could also occur due to some depressive symptoms.

Thanks for asking again

If you have 25 and 10 mg strengths then you can start taking 35 mg after about 2 weeks and then reduce to 25 mg for next two weeks and then for next two weeks go with half of 25 mg tablet to 20 mg doses and then finally 10 mg doses for 2-3 weeks followed by taking tablet on alternate basis for 1-2 weeks and then stopping completely. In this way you can go with smooth tapering. Alternatively you can start with taking 3/4 th tablet then half then 1/4th and then on alternate days followed by complete stoppage.

Both ways will prevent withdrawal symptoms. Take the drugs with proper prescription only.

they come in 25 mg and I take two at bed time which is 50 mg. They only come in capsule form and the lower dosage is 25 mg. Therefore, I can;t break it into half. I also take lorezepam ( ativan) 1 to 1 1/2 mg at bed time.

If you have capsular form of drug then you can take 25 mg and 10 mg doses to taper the dose for next two weeks and then with 25 mg doses. If possible try to get 10 mg tablets. Yes capsule cant be broken. Taking Lorazepam while tapering the dose of Doxepin will reduce the anxiety and will result in smooth withdrawal.

Even if you are not able to get 10 mg doses then you can start with taking 50 mg on one day and then 25 on next day and then alternately for next two weeks and after that reduce the dose to 25 mg and so on. Although it would not be completely smooth but still can be helpful in your case.

Discuss with your doctor before making any change in prescription.

Thanks for your prompt reply, Dr. seikhoo Bishnoi. I will discuss with my doctor about your suggestion. Is it really true that people over 65 should be careful with taking this medication because of their adverse side effect.
