Does Extreme Fatigue After Unprotected Oral Sex Indicate STD?

Question: Hello.. I really messed up and am worried about possible hiv and/or hep b/c. I had unprotected oral sex with a female sex worker (I was the giver) almost a month ago. I ended up with her discharge in my mouth and ran to spit it out in the sink. I should have gone to the ER but didn't. About 10 days later I felt extreme fatigue. I also had a night or two of night sweat, had inflamed glands for an hour only in the neck, had no fever, jaundice or true nausea, but for the last week I've had a very unusual weird pain/cramp in my stomach. It feels like nothing I have ever felt. More of a nuisance than a hard pain, but it has it moments of painfulness.. Almost like my stomach has an alien in it. My alt and ggt enzymes were both elevated in an unrelated blood draw only four days after this encounter. Alt only slightly, ggt about 70% above the high normal. I have tested for stds twice in the past two weeks including the hiv early detection two weeks after the encounter. All negative so far. I have read about symptoms of all stds and hiv and hep b scare me the most. I was given two antibiotics by the GP I saw last Tuesday.. One was a shot of rocephin and the other was a four pill one time only of something I'm not sure what. He said those would take care of possible chlymidia and/or gonoreah.. I had lower back pain and the stomach problem described earlier. I have an appointment next week with a liver specialist who I almost feel like I can't be completely straight with as my job depends on a good report from him.. Too long a story to get into but it involves screening for past alcohol abuse.. Sober from that for 3.5 years. Anyway, I'm scared to death and would love for you to be able to put me at ease, but also want your honest professional opinion. Thank you! (47 yr old male)

Thanks for posting your query.

First of all, transmission of STD/HIV/Hepatitis are low through oral sex, especially with a woman.
Through oral sex one can get other non-STD bacterial infections of throat and gut which can also produce such symptoms.
You had taken care already for Gonorrhea and Chlamydia by your GP. So nothing to worry about these. But these antibiotics has nothing to with viral infection, if any (rare). Tests for STD/HIV/Hepatitis after two weeks are so early and not very much conclusive.
You had already some risk factors for Hepatitis and liver problem from your habit of past alcoholism. So these symptoms are not necessarily due to STD/HIV.
Anyhow all your questions would better be answered by the tests to be done
4 weeks after the encounter as per the guidance of the Gastroenterologist (Liver Specialist). Get well soon.

Thank you Dr. Murugan! I was actually hoping it might be another virus that is not std related. What other vurus' are out there and how can they be treated? This killer knot in my stomach that is a varying degree of a fireball. How can it be treated? What are the odds of this causing long term damage and would it eventually get better with no treatment? Thanks for answering these follow ups and feel free to offer any advice on what to say to the doctor and/or take over the counter to help me.

There are so many bacteria other than STD causing bacteria like Streptococcus, Klebsiella, Shigella, Salmonella, Eschericia Coli etc. can cause an infection to produce various symptoms with throat and gut.
It is the job of your treating Physician to find the cause and treatment. There is a possibility of elimination of this infection spontaneously with your natural body immunity. There is nothing to fear about any permanent damage of your system.Anyhow it is better to have a consultation with your physician and he will direct you further and will relieve your symptoms.
