default
Posted on Mon, 22 Aug 2016
Question: currently I am planning for pregnancey and udner treatment.

My Gynac doctor has suggested me to take letrozole 5 mg this month from 3rd day and i am having that now.

Last month she suggested stiphere tablet, and i had it but I did not conceive( May be also because from my side and my husbands side
we could nto give our 100% )

But when i went through this tablet, many reviews say that, this tablet is not used for ovulution or should not be used when we plan fr pregnancey.

and it is mainly used for breast cancer.

So my concern is can I go ahead with this medicine. And in case I conceive this month with the help of this table,
hope ther is no birth defects in baby due to this tablet( as I go tto know when i searched more on this tablet).

Is it safe to plan pregnancy with this tablet letrozole ?

About my medical hsitory to hight light, I am also a neuro patient affected with seizures in intital level so having medicies
like oleptol 1200mg and Keppra 1250mg each day.

I am also having medicines like glycomet and folic acid.

and earlier already 2 times i had miscarriage 1st time during my 6th month, i got to know that baby is infected with Rubella and I was forced to go ahead
with volutary abortion.

2nd time during end of 2nd month it was missed abortion and I had to go for DNC.

since last 3 years I am seriously trying for baby.

Brief Answer:
letrozole used in clomifene resistant cases.

Detailed Answer:
Hello,
Thanks for the query to hcm,
Letrozole belongs to a class of medications known as aromatase inhibitors. Aromatase is an enzyme that is responsible for the production of estrogen in the body. Letrozole works by inhibiting aromatase thereby suppressing estrogen production.
Clomiphene citrate, on the other hand, blocks estrogen receptors. In both cases, the result is that the pituitary gland produces more of the hormones needed to stimulate the ovaries. These hormones, FSH and LH, can cause the development of ovulation in women who are anovulatory or increase the number of eggs developing in the ovaries of women who already ovulate.

This is the drug of choice in clomifene resistant cases and has been known to cause side effects on the foetus in 4.7% cases and hence to be used restrictively under supervision and hence once ovulation is confirmed it should be stopped.

Letrozole works based on its ability reduce estrogen levels. Low estrogen levels of any cause can cause a woman to have symptoms.Side effects that are similar to those seen with clomiphene citrate:
Hot flashes
Headaches
Breast tenderness
So you may experience them as a side effect but not necessarily. It is an acceptable option as you have bene trying for so long now.
Also oleptol and keppra both are safe in pregnancy.

Thank you.

Actually clome medicine worked well in my case during previous pregnancy.
But my gynac has now suggested Letrozole.
Last month I was asked to have stiphene tablet 100 mg , and this month Letrozole.
Cant we have stiphene 2nd month also? As I find Letrozole to little risky.

And I am having this 5mg tablet from my 3rd day till 7th day , so definitely before ovulation. so I believe it should not end with any problems with baby even if I conceive.

Thank you for your help

Brief Answer:
clomiphene can be used with increased dosages for 3 cycles atleast

Detailed Answer:
Hello,
A single cycle use of clomiphene is not considered a failure at all and it can very well be used for at least 3 cycles if the response to clomiphene was good, so you can continue clomifene even in this cycle with clomiphene 150mg from day 3 to day 7. Letrozole can actually be avoided for at least 2 more cycles.

Thank you for your reply.

I have following concerns also:-

1) Currently I am having glycomet 500 mg morning and night..
If i get conceived this month, then also do I need to continue with this medicine.
till which month of pregnancy do I need to continue?

2) Similarly with sustain tablet also. My gynac asks me to have tat tablet after ovalution. After conceiving till how many months do I need to have that?
As I have had 2 times miscarriages in past, I wanted to be carefull on all these things and really concerned.

3) Also my husband did a semen analysis just now.And the sperm count is 69 million cells / umm

Actively motile- 35%
Sluggish motile- 25%
Non motile-40%

And volume is 0.9 ml

Please let me know if these seems to be fine or it needs to be improved. If so what medicines can be taken?
Currently he is having Biorich tablet as the non motile percentage was 60% when tested 2 months back.

As you already know this month I am on Letrozole 5 mg along with glycomet and neuro tablets.
Thank you

Detailed Answer:
hello,
If you conceive during this cycle then both glycomet and susten tabs should be continued till 12 weeks of pregnancy , till placentation is complete and any chance of threatened miscarriage is averted.

Secondly biotech is only vitamins, so oligoasthenospermia should be first treated with a course of antibiotics for 10 days and then TAB QUINCIFER-M should be taken twice a day daily for next 2 months to increase the quality and quantity of sperms.

Detailed Answer:
Hello,
1. The sperm count and motility are low which makes him oligoasthenospermic. So ideally a semen culture should be done to rule out pus cells in semen which would indicate a chronic infection. In that case a course of antibiotics would have to be started for 10 days along with male fertility enhancing drugs with carnitine and Co-enzyme Q meds which shall improve the quality. So get a culture done for him.

2. While intercourse try keeping 2 pillows underneath your lower back so that sperms remains inside after ejacultion and stay in the position for at least 10-15 min to allow ejaculate to liquefy.
3. The life of sperms in reproductive tract is 72 hrs before they disintegrate.
