Posted on Thu, 5 Oct 2017
Question: Hi. My brother has been diagnosed with Metabolic Encephalophy. He fail a few months ago and broke his pelvis (which is healed) and he had a UTI during the same period which has been hard to clear up. He has been confused off and on and on Tuesday he was found in bed as non-responsive. Rushed him to hospital and did CTs. Not a stroke as first thought. His memory is spotty. He remembers people and things but seems confused in the present although today he knew it was his birthday.

Brief Answer:
Metabolic encephalopathy

Detailed Answer:
Hi, thanks for asking from HCM.

I can understand your concern. Metabolic encephalopathy respond to treatment if its cause is identified early and treated completely. In your brother's case, I think the cause was

- Urinary tract infection with/without sepsis
- electrolyte abnormalities

Treating these two causes will definitely help in better recovery. You mentioned that he has regained the consciousness. That is a good sign. The on/off memory loss will gradually subside. As soon as his electrolytes and infection will be controlled, he will gradual regain his cognitive functions. Please let me know his

- serum electrolyte, blood sugar, blood culture, kidney function test,

- serum calcium, arterial blood gas analysis.

If these reports are normal, it means he will recover fast. For cognitive function improvement, he can be started on some medication (if kidney and live function tests are normal)

- Citicoline
- Cerebroprotein hydroxylate
- piracetam
- donepezil

Also, try to support him. Talk good things and remind him about old good times.
Make him listen soothing music. Music enhances brain activity. Good diet is also needed, rich in protein.

Hope it will help him. If you have any doubts, do let me know.
Thanks. Take care.
