Can Hysterectomy And Liver Transplantation Causes Biploar Disorder?

default
Posted on Tue, 10 Jun 2014
Question: I have had RSD/CRPS for the past 20+ years and am also a liver transplant recipient in 2008. I have had many surgeries, laparotomies and a full hysterectomy over the last 30+ years. I was also diagnosed with having PTSD since the age of 9.

My question is can any or all of these cause a bi-polar condition of any type? My pain doctor wants me to see a psychiatrist to be tested to see whether or not I am bi-polar.

Is there any literature or web sites that can give me more information pertaining to bi-polar conditions, causes and treatments?

Detailed Answer:
HI
Thanks for using healthcare magic. I, Dr Abhishek Kapoor, would help you in this query.

Being a psychiatrist, i can tell you few things that would help you. Bipolar disorder, also known as bipolar affective disorder is a mental illness characterized by episodes of an elevated mood known as mania, usually alternating with episodes of depression. During mania an individual feels abnormally happy, energetic or excitable, but often makes poor decisions due to unrealistic ideas or poor regard of consequences. Duration of manic phase should be more than seven days.

Signs and symptoms of the depressive phase of bipolar disorder include persistent feelings of sadness, anxiety, guilt, anger, isolation or hopelessness, disturbances in sleep and appetite, fatigue and loss of interest in usually enjoyable activities, problems concentrating, loneliness, self-loathing, apathy or indifference, depersonalization, loss of interest in sexual activity, shyness or social anxiety, irritability, chronic pain, lack of motivation and morbid suicidal thoughts. A major depressive episode persists for at least two weeks, and may continue for over six months if left untreated.

There are lot of articles online that you can read. In case, you need further help, you can ask.

Dear Dr. XXXXXXX

Thank You for you reply. What I am asking is can having these extreme health issues and injury's be multiple surgeries ,having a liver transplant is not you normal everyday surgery with several complications after the surgery, and prior.

Can it be that the combinations of these health problems in the last 20 years make it look like symptom of bipolar. I have been seeing a psychiatrist for many year in assisting me with my medical problems .Having a hysterectomy with other female medical problems prior ,But having this surgery at 35 ,not having any children prior endometriosis. I have had 5 laparotomies 3 female 2 with my liver transplant which I had peritonitis following my transplant which than required a
second surgery antibiotic wash out with massive drugs I had to take. Prior on the transplant it self I had a allergic reaction to prograf 1 hour after my transplant rejecting my new liver .I was in ICU for 30 days . My doctors didn't tell me why or what had happened. My former husband of 22 years did. I was very upset that I was not told the truth about this transplant and my recovery. Was in the hospital for 6 months.

I am a former Flight Attendant of close to 30 years and designer for private aircrafts Fortune 500 companies. I come from a aviation back ground. My father also being a commercial pilot. At he are of 8 my family had moved from up state New York to a very small town in Oklahoma on the boarder of XXXXXXX and Ok.
My father was employed for a short time until his job was opening up with XXXXXXX Airlines in the XXXXXXX Ft. Worth area. He took a job prior to going to AA in Ardmore Oklahoma .
In April of 1966 he was killed due to the owner operator of this company was paying of the FAA and had a history of heart problems. There was over 700lbs on the yoke upon landing missed approach .Many thing went wrong. I witnessed his plane crash at night in highly poor weather. It was just my mother and I. My sister who is 12 years older was not there .She was in XXXXXXX in college however
she didn't show up until over 24 hours. This was a major aviation crash and the largest in the mid 60's for a major airline that took 98 plus people .On survivor.

If you interested you also can read on web XXXXXXX Flyers April 22,1966. My mother not realizing what she was doing was trying to help others in need .This was a very small town that didn't have the means for this type of crash and casualties. Announcements were made to the public to help move bodies those who had vans, pick up trucks ,station wagons. The town had to make morgues out of school gyms because of the body count. Unfortunately, my mother took me trailing behind her to see these morgues to try to reclaim my father flight bag and personal items. A long legal battle followed as my mother and I moved to XXXXXXX XXXXXXX
She just past away in 2012 at the age of 94 . Never remarried. My career started with Braniff International at 18 yrs. of age. This was the first there company had hired so young ,But I had all requirements 2 years of college and HS at 16 grad. Braniff while in training was the first to tell me what really happened regarding the crash, many of these instructors knew my father .I was told that they trained there pilots from this crash and the problems they had to make the plane crash.

For my mother this was a subject never spoken to me about. My former husband also a pilot said in speaking to all three of use we all had a different experience,
from this crash. My mother went from a stay at home mom to a working women in her mid 40's that had a very good life prior to this crash. A sister that held resentment toward me ,because she though she was the only child at 12. Than I came along. I was my fathers daughter when he was home I was always going to the airport he had me sitting on the right set of a DC-3 at the age of 3. I fell in love with these big flying machines which was still a high profile job to have in the bid 50's when I was born.

I am giving you this information because I don not have these high and lows .Yes sad ,afraid of these surgeries how they effected me. But in all the years that I have been seeing the same Psychiatrist never did he think I was bipolar. And to add my problems he just past away in a freak illness that ended in death which I had not been notified until several months after his death of this year.

My pain doctor who had spoken to my PHD question his knowledge because he was 70 when he died and that he thought the younger doctors could analysis my so called bipolar because they know what to look for. I took great offence at him questing this doctor that had gone threw transplant ,and other prior help issues.
This was a very respected doctor that had called him and tried to explain that he was dealing with a complicated patient that he felt I had suffered from PTSD from my fathers crash and what I witnessed.

I have RSD which my psychiatrist had worked with previous pain Management doctors on opioid therapy which dosage has not increased in 25 years, Now because this doctor is not around .I fell like he is looking for another medical problem he can tag on me. I don't have a problem going for an evaluation however one goes threw this with the University psychiatrist prior to being excepted into the transplant program. Everything that has been explained to you was told to there doctor. Every type of testing is done stress tests endless ex- ray's ,biopsies endoscope. colonoscopies for years. I was on UNOS transplant list over 4 years
It unheard of. I was dropped from the system. It just seems like as many doctors I had and still have one of them would have said something or noticed a behavior.

This doctor said because I laugh at something this was a sign of bipolar. I told him that I am know going back to get the new medication that's just been released for Hep-C .So it not enough that I have to go threw this again with diabetes which was caused by my transplant medication and wrong dosages .
And now tag on bipolar .

I realize this is a very long explanation of events. But could all of these medical problems and child experiences answer so question instead of criticizing a doctor that knew me well and died at 76 that he didn't think he had the knowledge to diagnose me?

Possible you could advise me what to do?

Regards, XXXXXXX XXXXXXX

Brief Answer:
No chances of bipolar in your case

Detailed Answer:
HI

Bipolar disorder is a psychological disorder and any physical or childhood trauma could lead to any psychological disorder in any person. In your case, the way, you have explained your symptoms, i do not think, you have bipolar disorder. For diagnosis of bipolar disorder, there should be manic or depressive episode. In your case, there may have been PTSD or depressive episode, but there was not clear cut manic episode and without manic episode, no one could diagnose you a case of bipolar disorder. So, chances of bipolar disorder is unlikely.

Only basis of self laughing, we can not make diagnosis of bipolar. There should also be number of other symptoms, which i have already mentioned in my first answer. Sometime, even drugs like corticosteroid, interferon etc cause behavioral changes in form of depression or psychosis, which are misdiagnosed by doctor.

In my opinion, you are not a case of bipolar disorder.

In case, you have more query, you can ask.
