What Does Swelling In The Right Hand Indicate?

My right hand has swollen and very uncomfortable, I am 72, I have various health issues like hearth disease, high blood pressure, high cholesterol, etc, under control with medications, could this be an indication of type of the start of organ failure, or is it some type of disease like lyme disease, I am outside allot in the summer,thanks

Swelling in the hand at 72 years indicate possible arthritis related matter or other soft tissue lesion.
I would recommend to consult in follow up with a photo picture of the hand.
