My dr. found 2 puffy spots on my neck , by thryroid. just wondering what can be ? I am usually always tired/sluggish. have no energy, loose it very fast during the day, I feel I always need to nap. Can you please give me an idea what this could be? Thank you...

The best thing is to get a thyroid function test and meet an endocrinologist. they will decide weather you require an ultrasound of neck or a nuclear scan based on examination and thyroid function results.
