Welcome to health care magic.

1. The long standing history of lump suggest benign pathology ( non cancerous ) most likely lipoma.
2.But present symptoms could be due to complications in the lump.
3.In this case very much recommended to get examined for better evaluation.
4.An ultrasound examination - will give the nature of the lump, its extensions, vascularity and source.
5.It could not be any thing serous - but you may need to get a small incision and a course of antibiotics post procedure.

Good luck.
Hope i have answered your query,
