What causes facial tics while suffering from congestive heart failure?

i noticed tis morning that my 86 year anther whom is is end stage congestive heart failure had what appeared to be a tremor in his face , I just observed him, him now resting comfortably in a nap but still appears to have this twitching or tremor, pleas help me understand what Im looking at, also he has mottling around his feet elbows and hands but still has a very hearty appetite

At the end stage or in the terminal stage the clinical appearance greatly varies and it is difficult to explain moreover your personal view of angle plays the great role if you are deeply attached with the patiently then the appearance would be much subtle, now the most common condition is hypoxia and it may cause the muscular twitching
