Hello!

Welcome on HCM!

I understand your concern and would explain that your symptoms could be related to a cervical spine disorder, like a bulging disc, which can cause both headache and shoulder pain.

So, I would recommend performing a cervical spine MRI study to exclude this possibility.

Regarding the treatments, it seems that you have already tried some of them without good results. Topramate and neurontin are both antiepileptic drugs which can be helpful in headache and shoulder pain. Anyway, I would recommend trying valproate for the pain or an antidepressant (amitryptiline).
But, first it is important exclude a cervical spine disorder, which could need surgical treatment.
You should discuss with your doctor on the above issues.

Hope to have been helpful!
Kind regards, Dr. Aida
