Suggest Therapy For Concussion

Dear Doctor, I fell walking UP the stairs two months ago. It was after one of my children s swim meets, my arms were full of all three of their things, and I tripped in my flip flops and landed straight on my forehead, right above my left eyebrow. I don t think I blacked out, but I was definitely seeing stars. I immediately vomited. I had a bump that made me slightly resemble the elephant man. (Woman in this case) it caused me to have the most atrocious black eye that took 5 weeks to resolve. I went to the dr on day 4 because I was super dizzy and in major pain still. The scan showed a concussion but no sign of a bleed. For about the past month, I have this sharp, stabbing pain that hits me, just above the bump that is still there. Takes my breath away a bit. I have three kids and a husband and just wonder if this is normal, as I need to be around for a lot longer! I still cannot touch the sight of the bump that remains...But I would expect that with the fall I took. This other weird pain that comes on is what worries me. Any insight you can provide is much appreciated. Thanks so much! :)

Hello!

Thank you for asking on HCM! I read your question carefully and understand your concern.

Your symptoms may be caused by a trigeminal neuralgia, which is an irritation of the trigeminal nerve, probably by the trauma.

Other causes that may mimic this clinical scenario would be a post-traumatic carotido-cavernous aneurysm, but it would also causes blurring vision and eye redness or protrusion.

The brain CT scan that you have performed has excluded a possible acute intracranial hemorrhage or any broken bones of the scalp and sinuses.

I recommend you to riconsult with your attending physician for a careful physical examination and perform a routine blood test and inflammation tests to exclude a possible inflammation or infection.

I would recommend you to start carbamazepine or lamotrigine if there are no contraindications to these drugs. These are two antiepileptic drugs very effective in nerve pain.
