Having Noticed A Hole On The Butt Crack

My 94 year old Mom has developed a hole in the crack of her butt. It is not red or draining, it is about as big around as my little finger. She says it s sore and itches. She spends most of her time in a recliner with the foot rest up because her feet swell badly if she keeps them down. She does not sleep on her back. As her caregiver I have no idea what to do. Your thoughts please.
