Brief Answer:
Orchiepidydimitis possible, do urine analysis and uroculture.

Detailed Answer:
Hello dear, welcome to Ask a doctor service.
I read your query and here is my advice.

I think you should do an urine analysis and uroculture. The urine test done 20 days before which comes out clear is not valuable now because the urine situation has probably changed.
The first thing we have to check is an urinary track infection or orchiepidydimitis.
The fact that you had symptoms and they become better by antibiotic usage and are reoccurring again this probably shows a repeated infections. Uroculture and antibiograme will determine the right treatment.

If the urine test and uroculture are negative then a testicle ultrasound should be done to check for varicocele (enlarged vein of testicle) which may cause the pain.
Waiting for test results you can use antinflamatory drugs such advil or acetaminophen for pain.

Hope I have answered the question.
Let me know if I can assist you further.
