Brief Answer:
It could be UTI with Epididymitis.

Detailed Answer:
Hi.
Do you have any other diseases like Diabetes or Hypertension for which you are on medication?
Is there any swelling in the testicles or any burning while passing urine? Do you have fever intermittently? If these things are there then you possibly have Urinary tract infection and you should get a Urine Culture test done.
This will show up whether there is any infection and what antibiotics should be started to treat the infection.
If there is significant swelling then an Ultrasound of the Scrotum would be required
It would be best if you can get examined by your Doctor or Surgeon. He would be able to diagnose the problem easily.
Take care and do let me know the answers to above questions to shed more light onto the problem.
