What Causes Pain In The Lower Right Abdomen Radiating To The Back?

default
Posted on Fri, 16 Sep 2016
Question: For several weeks, my stomach has been sore to the touch and swollen. I have a slight stabbing pain in the lower right side of my stomach, that comes and goes. Often, I can only eat once a day because of the feeling of fullness that doesn't go away. Pain often radiates to my back. Some days, I have very loose stools. Leg and arm muscles are sore and I am retaining a lot of fluid.

Brief Answer:
Needs clinical evaluation and investigations as mentioned.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/46 - stomach is sore to touch and is swollen - several weeks - pain on right lower that comes and goes - radiates to back - feeling of fullness - eat once a day - very loose stools some days - leg and arm muscles sore - retaining a lot of fluid ...
As per the history you have provided I would think of the following probable reasons:
Appendicitis, particularly retrocecal
Typhlitis, inflammation of cecum
With all secondary symptoms as you have mentioned.
Since you are suffering from several weeks, it isbetter to get the following things done asap:
Tests of stool for 3 consecutive days
Blood- CBC, Sugar, Thyroid and liver function tests.
Urine tests
High resolution ultrasonography and preferably Contrast enhanced CT scan of the abdomen as it shows more than the ultrasound.
Clinical evaluation by your Doctor, preferably a General Surgeon.

These will give you the most proper diagnosis and a plan for further management to get a probable cure.
Please feel free to ask for further relevant queries if you feel that there is a gap of communication.

Thank you for your response. I have an appointment scheduled for next week.

Another symptom I failed to mention:
If I eat more than once a day or if I eat too much in a sitting, the pain in my stomach and middle back is nearly unbearable and I can't find relief for a couple of days. Since I rarely feel hunger anymore, I make myself eat at least once a day and avoid situations where food is involved.
I don't know if this helps in diagnosis, but thought I would mention it..

Brief Answer:
Enterography or enteroclysis/ CT scan of abdomen.

Detailed Answer:
Thanks for your feedback.

The additional information indicates that you may have sub-acute intestinal obstruction and needs to be ruled out.

Contrast enhanced CT scan with enterography or barium mean and follow through helps for this.

Please discuss about this with your General Surgeon to get it done and get a proper diagnosis.

All the best. Please give feedback about the reports of the investigations.
